眼前节OCT在闭合性眼球损伤患者眼前节病变治疗中的应用

摘　　要：目的探讨眼前节光学相干断层扫描(AS-OCT)在闭合性眼外伤患者眼前节病变中的应用。方法回顾性病例系列研究。收集郑州大学第一附属医院2020年8月至2021年7月闭合性眼外伤81例(86眼)的临床资料。所有患者均行视力、眼压、裂隙灯显微镜检查及AS-OCT扫描, 并根据伤情对症治疗。收集患者的一般资料, 包括年龄、性别、损伤类型、损伤程度和损伤区域, 分析影响闭合性眼外伤患者效果的危险因素。结果闭合性眼外伤的AS-OCT表现包括:角膜损伤86眼(100.00%, 86/86)、前房积血46眼(53.49%, 46/86)、前房角后退19眼(22.09%, 19/86)、睫状体脱离24眼(27.91%, 24/86)、晶状体脱位14眼(16.28%, 14/86)、虹膜损伤11眼(12.79%, 11/86)。单因素Logistic回归分析结果显示:前房角后退(OR=1.08,P=0.014)、睫状体脱离(OR=5.83,P=0.006)、晶状体脱位(OR=6.21,P<0.001)及虹膜损伤(OR=4.28,P<0.001)是影响闭合性眼外伤患者视力预后的危险因素。结论眼前节OCT检查可为闭合性眼外伤眼前节病变的诊断和治疗提供准确的资料。Objective To investigate the application of anterior segment optical coherence tomography(AS-OCT)in anterior segment in patients with closed ocular trauma.Methods This was a retrospective case series study.The clinical data of 86 eyes of 81 cases with closed ocular trauma in the First Affiliated Hospital of Zhengzhou University from Aug.2020 to Jul.2021 were collected,and all patients received visual acuity,IOP,slit lamp microscopy examination and AS-OCT scanning.All cases received symptonmatic treatment according to the injury.The general data of patients,including age,gender,injury type,injury degree and injury area were collected,and the risk factors affecting the efficacy of patients with closed ocular trauma were analyzed.Results AS-OCT manifestations of closed ocular trauma included:corneal injury in 86 eyes(100.00%,86/86),hyphema in 46 eyes(53.49%,46/86),anterior chamber angle recession in 19 eyes(22.09%,19/86),ciliary body detachment in 24 eyes(27.91%,24/86),lens dislocation in 14 eyes(16.28%,14/86)and iris injury in 11 eyes(12.79%,11/86).The results of univariate logistic regression analysis showed that angle regression(OR=1.08,P=0.014),ciliary body detachment(OR=5.83,P=0.006),lens dislocation(OR=6.21,P<0.001)and iris injury(OR=4.28,P<0.001)were risk factors affected the visual prognosis of patients with closed ocular trauma.Conclusion AS-OCT is a very good and accurate examination method for diagnosis and treatment of anterior segment lesions in closed ocular injury.
