养阴解毒汤对气阴两虚型晚期鼻咽癌放化疗患者免疫功能和癌因性疲乏的影响

摘　　要：目的:观察养阴解毒汤对气阴两虚型晚期鼻咽癌放化疗患者的疗效以及免疫功能和癌因性疲乏的影响。方法:将94例晚期鼻咽癌患者随机分为观察组和对照组,每组47例。对照组患者予放化疗治疗;观察组患者在对照组基础上口服养阴解毒汤。两组连续治疗9周,并随访6个月。比较两组疗效、不良反应、外周血淋巴细胞亚群、癌因性疲乏评分。结果:观察组患者总有效率为87.23%,与对照组65.96%比较,差异有统计学意义(P<0.05);观察组治疗期间白细胞减少、中性粒细胞减少、血红蛋白减少、胃肠道反应、口腔黏膜炎、放射性皮炎的发生率与对照组比较均下调,并且两组口腔黏膜炎发生率比较差异有统计学意义(P<0.05);观察组患者治疗后的CD3^(+)、CD4^(+)高于对照组CD8^(+)低于对照组(P<0.05)。随访6个月后,观察组患者CD3^(+)、CD4^(+)、CD8^(+)较对照组均明显改善(P<0.05)。治疗后,对照组患者躯体疲乏、情感疲乏、认知疲乏评分明显增加,且观察组以上评分明显低于对照组(P<0.05)。结论:对气阴两虚型晚期鼻咽癌放化疗患者予养阴解毒汤干预疗效显著,可有效减轻放化疗所引起的不良反应、免疫功能损伤以及癌因性疲乏。Objective:To observe the efficacy of Yangyin Jiedu decoction combined with radiotherapy and chemotherapy in treating advanced nasopharyngeal carcinoma patients with syndrome of qi and yin deficiency and its effect on immune function and cancer-related fatigue.Methods:A total of 94 patients with advanced nasopharyngeal carcinoma were selected and divided into observation group and control group according to random number table method,47 cases in each group.Control group received radiotherapy and chemotherapy,observation group was given Yangyin Jiedu decoction orally on the basis of control group.Both groups were treated for 9 weeks and followed up for 6 months.Efficacy,adverse reactions,peripheral blood lymphocyte subsets and scores of cancer-induced fatigue were compared between the two groups.Results:The total effective rate was 87.23%for observation group and 65.96%for control group,and the difference was statistically significant(P<0.05).The incidence of leukopenia,neutropenia,hemoglobin reduction,gastrointestinal reaction,oral mucositis and radioactive dermatitis in the observation group were lower than those in the control group,and the difference in the incidence of oral mucositis between the two groups was statistically significant(P<0.05).After treatment,CD3^(+)and CD4^(+)for observation group were evidently higher as compared with control group,while CD8^(+)was lower than that in control group(P<0.05).After 6 months of follow-up,CD3^(+),CD4^(+)and CD8^(+)in observation group were significantly improved compared with control group(P<0.05).After treatment,the scores of physical fatigue,emotional fatigue and cognitive fatigue in control group were significantly increased,and those for observation group were evidently lower than those in control group(P<0.05).Conclusion:The intervention of Yangyin Jiedu decoction on advanced nasopharyngeal carcinoma patients with qi and yin deficiency has a significant effect,which can effectively reduce the adverse reactions,immune function damage and cancer-induced fatigue
