二参芍麦汤治疗慢性萎缩性胃炎(胃阴不足型)的临床研究及镜像学改变

摘　　要：目的:观察二参芍麦汤治疗胃阴不足型慢性萎缩性胃炎(Chronic Atrophic Gastritis,CAG)的临床疗效、镜像学改变及对其安全性进行系统评价。方法:收集符合胃阴不足型CAG诊断标准的患者作为观察对象,随机分为治疗组和对照组。治疗组以二参芍麦汤治疗,对照组以养胃舒胶囊治疗。观察两组患者的临床症状、胃镜及病理疗效的改变,同时进行安全性检测。结果:两组治疗后症状积分对比,治疗组优于对照组,差异具有统计学意义(P<0.05)。治疗组中医证候疗效总有效率优于对照组。治疗后两组患者胃镜下黏膜改善情况均有改善,且二参芍麦汤组在改善内镜下胃黏膜情况方面优于对照组。对照组治疗前后萎缩病理积分的差异具有显著统计学意义(P<0.05)。两组间治疗后各病理积分比较,差异具有统计学意义(P<0.05)。结论:二参芍麦汤可以缓解胃阴不足型CAG患者的症状、体征,改善胃黏膜萎缩、肠上皮化生的程度,从而阻断病情发展,提高患者的生活质量。Objective:To observe the clinical efficacy,mirrorological changes of the Ershen Shaomai decoction(二参芍麦汤)on chronic atrophic gastritis of the gastric Yin(阴)-deficiency type and systematic evaluation of its safety.Methods:Patients who met the diagnostic criteria of chronic atrophic gastritis of the gastric Yin-deficiency type were collected as observation objects and were randomly divided into the treatment group and the control group,with 30 cases in each.The treatment group was treated with the Ershen Shaomai decoction,and the control group was treated with Yangweishu capsules(养胃舒胶囊).The changes of clinical symptoms,gastroscopy and pathological efficacy in 2 groups were observed,and the safety tests were carried out simultaneously.Results:The symptom scores and total efficacy in the treatment group were better than the control group(P<0.05).After treatment,the improvement of mucosa under gastroscopy was improved in both groups,and in the treatment group was better.The pathological scores of atrophy in the control group before and after treatment showed a statistically significant difference(P<0.05),and the pathological scores showed a statistically significant difference between groups(P<0.05).Conclusion:This clinical study further confirmed that the Ershen Shaomai decoction can relieve the symptoms and signs of chronic atrophic gastritis of the gastric Yin-deficiency type,ameliorate the degree of gastric mucosa atrophy and intestinal metaplasia,thus preventing the development of the disease and improving the quality of life of patients.
