基于数据挖掘的卢秉久教授辨治肝纤维化肝郁脾虚证用药规律分析

摘　　要：目的基于数据挖掘科研方法分析卢秉久教授辨治肝纤维化(hepatic fibrosis,HF)属肝郁脾虚证的用药规律,为临床提供参考。方法收集卢秉久教授2014年12月15日—2020年12月15日在辽宁中医药大学附属医院肝病门诊系统中213例HF肝郁脾虚证病例数据及首诊处方213首,应用统计学软件进行高频药物复杂网络分析、聚类分析等。结果对所选213首处方进行分析,涉及中药149味,药性多为甘温,归经多为脾、肝、肺三经。从药物类别来看,补虚药、理气药、活血祛瘀药应用居多。关联规则分析得出常用药对及药物组,高频药物聚类分析共聚5大类。结论卢师治疗HF肝郁脾虚证患者以补虚药与理气药为基准,以达到健脾益气、调畅全身气机之功,配合其他类别药物治疗标证,并佐入行气破血之品,防止疾病传变。总的用药特点可概括为补中有行,散中有补,标本兼治,既病防变。Objective Based on data mining scientific research method,the drug use rules of Professor LU Bingjiu's differentiation and treatment of hepatic fibrosis(HF)belong to liver depression and spleen deficiency syndrome,so as to provide reference for clinical practice.Methods Data of 213 HF patients with liver depression and spleen deficiency syndrome and 213 prescriptions were collected from the liver disease outpatient system of the affiliated hospital of Liaoning university of traditional Chinese medicine from December 15,2014 to December 15,2020.High-frequency drug complex network analysis and cluster analysis were performed by statistical software.Results The 213 prescriptions were analyzed,and 149 kinds of traditional Chinese medicine were involved,most of which were sweet and warm,and most of which were channelized to spleen,liver and lung.From the point of view of drug category,the application of drugs for tonifying deficiency,regulating Qi,promoting blood circulation and removing blood stasis was in the majority.Association rule analysis of common drug pairs and drug groups,high frequency drug cluster analysis copolymerization of five categories.Conclusion In the treatment of patients with HF syndrome of stagnation of liver and spleen and deficiency of spleen,Dr.LU takes drugs to replenish deficiency and regulate Qi as the basis,so as to strengthen the spleen and replenish Qi,and smooth the whole body's Qi mechanism.In combination with other kinds of drugs to treat the standard syndrome,he also adds medicines to promote Qi and break blood,so as to prevent the transmission of disease.The general characteristics of medication can be summarized as:there are lines in tonic,there are lines in powder,treat both symptoms and root causes,and prevent disease from changing.
