CK2β与E-cadherin在宫颈癌中的表达及对患者3年生存率的预测价值

摘　　要：探讨蛋白激酶CK2β与E-钙黏蛋白(E-cadherin)在宫颈癌中的表达及对患者3年生存率的预测价值。选择2013年3月—2017年2月江苏大学第四附属医院妇科收治的60例宫颈癌患者,应用免疫组化法检测宫颈癌组织和旁癌组织E-cadherin与CK2β的阳性表达情况,分析二者与临床病理参数的关系,并探讨其对预后的评估价值。结果显示:宫颈癌组织CK2β蛋白阳性率明显高于旁癌组织,而宫颈癌组织E-cadherin阳性率明显低于旁癌组织(P<0.05);宫颈癌组织E-cadherin\,CK2β蛋白与临床分期、淋巴结转移及分化程度等标志物相关(P<0.05),而与肿瘤直径、病理类型、脉管瘤栓、深肌层浸润及宫旁浸润等指标无关(P>0.05);单因素分析结果显示,宫颈癌患者3年生存率与肿瘤直径、临床分期、分化程度、CK2β蛋白表达和E-cadherin表达等标志物差异有统计学意义(P<0.05);Cox多因素分析显示,临床分期(Ⅲ~Ⅳ)(OR=1.335,95%CI:1.029~1.733)、分化程度(低分化)(OR=2.018,95%CI:1.034~3.937)、淋巴结转移(OR=2.770,95%CI:2.093~3.667)、CK2β蛋白阳性表达(OR=2.563,95%CI:2.102~3.124)和E-cadherin阳性表达(OR=0.415,95%CI:0.339~0.508)是宫颈癌患者3年生存率的独立影响因素。研究表明E-cadherin与CK2β表达与宫颈癌淋巴结转移、临床分期和分化程度相关,且E-cadherin与CK2β是影响宫颈癌患者3年生存率的独立预后因素。The purpose of this paper is to study expression of CK2βand E-cadherin in cervical cancer and the predictive value for 3-year survival rate of patients.60 cases of patients with cervical cancer from March 2013 to February 2017 in Zhenjiang Fourth People's Hospital were selected as study object.The positive expression of cervical cancer tissue of E-cadherin and CK2βwas detected by immunoglostification,the relationship between the two and clinical pathological parameters was analyzed,and the evaluation value of prognosis was discussed.The positive rate of CK2βprotein in cervical cancer tissue is significantly higher than that of side cancer tissue,while the positive rate of E-cadherin protein in cervical cancer tissue is significantly lower than that of side cancer tissue(P<0.05).The results of single-factor analysis showed that the three-year survival rate of cervical cancer patients was statistically significant with the index of tumor diameter,clinical staging,degree of differentiation,CK2βprotein expression and E-cadherin protein expression(P<0.05).Cox multivariate analysis showed that clinical stage(Ⅲ~Ⅳ)(OR=1.335,95%CI:1.029~1.733),differentiation degree(low differentiation)(OR=2.018,95%CI:1.034~3.937),lymph node metastasis(OR=2.770,95%CI:2.093~3.667),the positive expression of CK2βprotein(OR=2.563,95%CI:2.102~3.124)and E-cadherin protein(OR=0.415,95%CI:0.3390.508)were independent influencing factors of 3-year survival rate in patients with cervical cancer.The results showed that the expression of E-cadherin and CK2βwas correlated with lymph node metastasis,clinical stage and differentiation of cervical cancer,and E-cadherin and CK2βwere independent prognostic factors affecting the 3-year survival rate of cervical cancer patients.
