摘  要:目的探讨外周血中性粒细胞与淋巴细胞的比值(NLR)及单核细胞与淋巴细胞的比值(MLR)与晚期非小细胞肺癌患者疗效及预后的关系。方法选择2016年至2021年在南京医科大学第一附属医院就诊的晚期非小细胞肺癌患者117例,均接受一线化疗或联合分子靶向药物治疗。每例患者接受4个周期的治疗后评价疗效。检测患者治疗前3天内NLR及MLR的数值。分析NLR、MLR与晚期非小细胞肺癌患者疗效及预后的关系。结果根据疗效评价,将患者分为疾病进展组(40例)和疾病控制组(77例)。疾病进展组的NLR及MLR分别为4.27±2.46和0.44±0.19,高于控制组的2.61±1.32和0.34±0.16(P<0.05);通过ROC曲线分析,NLR、MLR预测晚期非小细胞肺癌患者治疗疗效的最佳临界值为3.41、0.31,将患者分为高NLR组(40例)、低NLR组(77例)及高MLR组(68例)、低MLR组(49例)。低NLR组及低MLR组的治疗疗效明显好于高NLR组及高MLR组(P<0.05)。根据Pearson相关性分析显示,NLR与MLR之间呈正相关关系(r=0.84,P<0.05)。结论外周血NLR、MLR的高低与晚期非小细胞肺癌患者治疗疗效及预后相关,治疗前低NLR或低MLR的患者疗效明显好于高NLR或高MLR的患者。检测外周血中NLR及MLR值,可辅助评价患者的用药疗效及评估预后。Objective To investigate the relationship between neutrophil and lymphocyte ratio(NLR),as well as monocyte and lymphocyte ratio(MLR)in peripheral blood and curative effect and prognosis of patients with advanced non-small cell lung cancer.Methods A total of 117 patients with advanced non-small cell lung cancer who were treated in the First Affiliated Hospital of Nanjing Medical University from 2016 to 2021were selected for the study,and all patients received first-line chemotherapy or combined molecular targeted drug therapy.Each patient received 4 cycles of treatments and the curative effect was evaluated.We detected values of NLR and MLR within 3 days before treatment.We then analyzed the relationship between NLR,MLR and the efficacy and prognosis of patients with advanced non-small cell lung cancer.Results According to the efficacy evaluation,these patients were divided into a disease progression group(40 cases)and a disease control group(77 cases).NLR and MLR in the disease progression group were 4.27±2.46 and 0.44±0.19,respectively,which were higher than 2.61±1.32 and 0.34±0.16 in the control group(P<0.05).Through ROC curve analysis,the optimal critical values of NLR and MLR for predicting the therapeutic efficacy of patients with advanced non-small cell lung cancer were 3.41 and 0.31.With that,patients were divided into the high NLR group(40 cases),low NLR group(77 cases),high MLR group(68 cases)and low MLR group(49 cases).The therapeutic effect of the low NLR group and low MLR group was significantly better than that of the high NLR group and high MLR group(P<0.05).According to the Pearson correlation analysis,there was a positive correlation between NLR and MLR(r=0.84,P<0.05).Conclusion Levels of NLR and MLR in peripheral blood are related to the therapeutic efficacy and prognosis of patients with advanced non-small cell lung cancer.The efficacy of patients with low NLR or low MLR before treatment is significantly better than that of patients with high NLR or high MLR.Detection of NLR and MLR values i
