肺癌脑转移瘤的不同伽玛刀治疗方式近期疗效观察

摘　　要：目的探究伽玛刀单次与分次治疗肺癌脑转移瘤的临床疗效,评估其治疗价值。方法50例肺癌脑转移瘤患者,随机分为观察组和对照组,各25例。对照组进行伽玛刀单次治疗,观察组进行伽玛刀分次治疗。比较两组肿瘤局控率、1年生存率、急性放疗反应。结果治疗后,观察组肿瘤局控率及1年生存率分别为88.0%、84.0%,均显著高于对照组的60.0%、56.0%,差异有统计学意义(P<0.05)。观察组1、2级放射性中枢神经系统损伤发生率分别为20.0%、16.0%,均低于对照组的48.0%、44.0%,差异有统计学意义(P<0.05)。结论伽玛刀分次治疗策略可以明显增加肺癌脑转移瘤的治疗效果,而且也能够增加患者的生存率,同时减少急性放疗反应的发生率,其应用价值极大。Objective To investigate the clinical efficacy of single-stage and fractionated gamma knife radiosurgery for brain metastases from lung cancer,and evaluate its therapeutic value.Methods A total of 50 patients with brain metastases from lung cancer were randomly divided into observation group and control group,with 25 cases in each group.The control group received single-stage gamma knife radiosurgery,and the observation group received fractionated gamma knife radiosurgery.The local tumor control rate,1-year survival rate and acute radiotherapy response were compared between the two groups.Results After treatment,the local tumor control rate and 1-year survival rate of the observation group were 88.0% and 84.0%,respectively,which were significantly higher than 60.0% and 56.0% of the control group,and the differences were statistically significant(P<0.05).The incidence rates of grade 1 and 2 radiation central nervous system injury in the observation group were 20.0% and 16.0%,respectively,which were lower than 48.0% and 44.0% in the control group,and the difference was statistically significant(P<0.05).Conclusion Fractionated gamma knife radiosurgery can obviously improve the therapeutic effect of brain metastases from lung cancer,and but also can increase the survival rate of patients,while reducing the incidence of acute radiotherapy reactions,which is of great application value.
