骨髓形态学检查与急性早幼粒白血病的相关性研究

摘　　要：目的:探讨骨髓形态学检查与急性早幼粒白血病的相关性。方法:选取2018年7月—2022年4月在本院接受治疗的200例急性早幼粒白血病(APL)患者进行研究,采集200例患者初诊、治疗第14天、治疗第28天、治疗第84天的骨髓细胞涂片,并对复检报告和原始报告进行对比,分析骨髓形态学检查与急性早幼粒白血病的关系。结果:2571份骨髓涂片标本在疑似APL骨髓涂片中的检出率为4.98%。128份APL骨髓涂片标本初诊确诊率为7.81%,复查确诊率为89.84%,总准确率为97.66%;有3份发现骨髓稀释,无法确诊。初诊10份确诊APL骨髓涂片标本中的CR、PR、NR和复发率分别为80.00%、20.00%、0.00%、0.00%;复查118份APL骨髓涂片标本中的CR、PR、NR和复发率分别为67.80%、32.20%、0.00%、0.85%。结论:骨髓形态学检查与APL存在一定关联,其检验结果可鉴别诊断APL,同时还能有效评估其预后,为治疗方案的制定提供参考依据。Objective:To explore the correlation between bone marrow morphological examination and acute promyelocytic leukemia.Methods:200 patients with acute promyelocytic leukemia(APL)who were treated in our hospital from July 2018 to April 2022 were selected for research.Bone marrow cell smear,and the re-examination report and the original report were compared to analyze the relationship between bone marrow morphological examination and acute promyelocytic leukemia.Results:The detection rate of 2571 bone marrow smear specimens in suspected APL bone marrow smear was 4.98%.The 128 APL bone marrow smear specimens had an initial diagnosis rate of 7.81%,a re-examination diagnosis rate of 89.84%,and an overall accuracy rate of 97.66%;3 cases showed bone marrow dilution,which could not be diagnosed.The rates of CR,PR,NR and recurrence in 10 confirmed APL bone marrow smear specimens were 80.00%,20.00%,0.00%,and 0.00%,respectively;CR,PR,NR and recurrence in 118 APL bone marrow smear specimens were re-examined The rates were 67.80%,32.20%,0.00%,and 0.85%,respectively.Conclusion:There is a certain relationship between bone marrow morphological examination and APL,and its test results can differentiate and diagnose APL,and at the same time can effectively evaluate its prognosis,and provide a reference for the formulation of treatment plans.
