摘 要:目的:观察桃红四物汤对创伤性股骨头坏死模型大鼠病理形态与BMP2、Smad1/5/8、Runx2蛋白及mRNA表达的影响,并探讨可能作用机制。方法:将35只SD大鼠采用完全随机分组法分成空白组、模型组、桃红四物汤高剂量组、桃红四物汤中剂量组、桃红四物汤低剂量组,每组7只。除空白组外,其余各组大鼠均采用股骨头血供剥离法造模;造模4周后,每组随机抽取1只大鼠处死,取术侧股骨以大体观察验证造模成功;验证造模成功后,桃红四物汤低、中、高剂量组大鼠分别灌胃给予相应药物,空白组及模型组大鼠灌胃等容量生理盐水,2次/d,连续灌胃6周。取大鼠股骨头骨组织,采用HE和Masson染色观察骨组织细胞病理情况,采用Western blotting检测骨组织BMP2、Smad1、Smad5、Smad8、Runx2蛋白表达情况,采用RT-PCR检测细胞内BMP2 mRNA、Smad1 mRNA、Smad5 mRNA、Smad8 mRNA、Runx2 mRNA表达情况。结果:与空白组比较,模型组大鼠股骨头病理形态发生破坏和病变,钙流失、骨结构破坏增加及造血细胞减少,骨组织中BMP2、Smad1/5/8、Runx2蛋白相对表达量降低,组织细胞中的BMP2 mRNA、Smad1/5/8 mRNA、Runx2 mRNA相对表达量下调,差异均有统计学意义(P<0.05)。与模型组比较,桃红四物汤低、中、高剂量组大鼠股骨头钙化组织、骨小梁、造血细胞等病理形态均有改善;与模型组比较,桃红四物汤高剂量组大鼠股骨头空骨陷窝率、IOD/Area比值明显降低(P<0.05);与模型组比较,桃红四物汤低、中、高剂量组大鼠股骨头骨组织BMP2、Smad1/5/8、Runx2蛋白相对表达量明显升高,组织细胞中的BMP2 mRNA、Smad1/5/8 mRNA、Runx2 mRNA相对表达量上调,差异均有统计学意义(P<0.05)。桃红四物汤低、中、高剂量组大鼠股骨头病理形态变化改善程度、骨组织BMP2、Smad1/5/8、Runx2蛋白相对表达量及组织细胞中的BMP2 mRNA、Smad1/5/8 mRNA、Runx2 mRNA相对表达量的升高呈现明显量Objective:To observe the effect of Taohong Siwu Decoction on the pathological morphology and protein and mRNA expression of BMP 2,Smad1/5/8,Runx2 in rats with traumatic osteonecrosis of femoral head,and explore the possible mechanism.Method:35 SD rats were randomly divided into blank group,model group,Taohong Siwu Decoction high-dose,middle-dose and low-dose groups,with 7 rats in each group.Except for the blank group,the rats in the other groups were modeled by the femoral head blood supply stripping method Four weeks after modeling,one rat was randomly selected from each group and killed.The femur of the operation side was taken to verify the success of modeling by gross observation.After the successful establishment of the model,rats in the low,medium and high dose groups of Taohong Siwu Decoction were given the corresponding drugs by gavage respectively.Rats in the blank group and model group were given the same volume of normal saline by gavage,twice a day,for 6 weeks.The bone tissue of rat femoral head was taken,and the cellular pathology of bone tissue was observed by HE and Masson staining.The expression of BMP2,Smad1,Smad5,Smad8 and Runx2 protein in bone tissue was detected by Western blotting,and the expression of BMP2 mRNA,Smad1 mRNA,Smad5 mRNA,smad8 mRNA and Runx2 mRNA in cells was detected by RT-PCR.Result:Compared with the blank group,the pathological morphology of the femoral head in the model group was destroyed and pathological changes,calcium loss,bone structure destruction increased and hematopoietic cells decreased,the relative expression of BMP 2,Smad1/5/8 and Runx2 protein in bone tissue decreased,and the relative expression of BMP 2 mRNA,Smad1/58 mRNA and Runx2 mRNA in tissue cells were down regulated.The difference was statistically significant(P<0.05).Compared with the model group,the pathological morphology of calcified tissue,trabecular bone and hematopoietic cells of the femoral head in the low,medium and high dose groups of Taohong Siwu Decoction were improved.Compared with the model g
