MSCT联合肿瘤标志物检测诊断卵巢癌的价值

摘　　要：目的探讨多层螺旋电子计算机断层扫描(MSCT)联合肿瘤标志物检测诊断卵巢癌的临床价值。方法回顾性分析2017年1月至2021年1月济南市第五人民医院收治的85例卵巢肿瘤手术患者的临床资料,根据病理结果分为良性组(43例)与卵巢癌组(42例)。所有患者均接受MSCT扫描及肿瘤标志物[糖类抗原125(CA125)、糖类抗原19-9(CA19-9)、甲胎蛋白(AFP)、癌胚抗原(CEA)]检查。比较两组患者肿瘤标志物水平。分析肿瘤标志物、MSCT、MSCT联合肿瘤标志物诊断卵巢癌的效能。结果卵巢癌组CA125、CA19-9、AFP、CEA水平均高于良性组(P均<0.05)。肿瘤标志物诊断卵巢癌的符合率为65.88%(56/85),MSCT的诊断符合率为78.82%(67/85),MSCT联合肿瘤标志物的诊断符合率为88.24%(75/85),MSCT联合肿瘤标志物与肿瘤标志物、MSCT单独诊断的一致性一般(Kappa=0.46、0.57,P均<0.05)。MSCT联合肿瘤标志物诊断卵巢癌的灵敏度为90.48%(38/42),特异度为86.05%(37/43),阳性预测值为82.61%(38/46)。MSCT联合肿瘤标志物诊断卵巢癌的曲线下面积为0.887,高于MSCT(0.803)与肿瘤标志物(0.534)单独诊断。结论MSCT联合肿瘤标志物检测诊断卵巢癌的符合率较高,良、恶性卵巢肿瘤患者血清肿瘤标志物表达水平存在明显差异。Objective To investigate the clinical value of multi-slice spiral computed tomography(MSCT)combined with tumor markers in the diagnosis of ovarian cancer.Methods The clinical data of 85 patients with ovarian tumors treated in Jinan Fifth People’s Hospital from January 2017 to January 2021 were analyzed retrospectively.According to the pathological results,they were divided into benign group(43 cases)and ovarian cancer group(42 cases).All patients underwent MSCT scanning and detection of tumor markers including carbohydrate antigen 125(CA125),carbohydrate antigen(CA19-9),alpha fetoprotein(AFP)and carcinoembryonic antigen(CEA).The levels of tumor markers in the two groups were compared.And the efficacy of tumor markers,MSCT,and MSCT combined with tumor markers in the diagnosis of ovarian cancer were analyzed.Results The levels of CA125,CA19-9,AFP and CEA in the ovarian cancer group were higher than those in the benign group(all P<0.05).The coincidence rate of tumor markers in the diagnosis of ovarian cancer was 65.88%(56/85),the coincidence rate of MSCT was 78.82%(67/85),and the coincidence rate of combination of tumor markers with MSCT was 88.24%(75/85).MSCT combined with tumor markers has common consistency with the tumor markers and MSCT(Kappa=0.462,0.572;both P<0.05).The sensitivity of MSCT combined with tumor markers in the diagnosis of ovarian cancer was 90.48%(38/42),and the specificity was 86.05%(37/43),and the positive predictive value was 82.61%(38/46).The area under the curve(AUC)of MSCT combined with tumor markers in the diagnosis of ovarian cancer was 0.887,higher than that of MSCT(AUC=0.803)and tumor markers(AUC=0.534).Conclusions MSCT combined with tumor markers has a high coincidence rate in the diagnosis of ovarian cancer.There are significant differences in the expression of serum tumor markers between patients with benign and malignant ovarian tumors.
