摘　　要：[目的]探讨放大内镜联合蓝激光成像技术(ME-BLI)在胃早期肿瘤性病变患者中的应用价值。[方法]选取行胃镜检查的88例可疑胃早癌患者为研究对象,所有患者先行普通白光内镜(WLE)检查,然后行ME-BLI检查并靶向活检,根据病理结果分为早癌组(21例)和非早癌组(67例)。比较2组的基线资料和ME-BLI征象,采用受试者工作曲线下面积(AUC)评价其对胃早癌的诊断价值。[结果]早癌组的边界线(DL)、不规则的黏膜微血管(MV)、不规则的表面结构(MS)的发生率明显高于非早癌组,差异有统计学意义(P<0.05)。DL联合MV、MS对胃早癌的AUC为0.875,具有较高的诊断价值。[结论]ME-BLI是胃早癌的重要诊断方法,DL联合MV、MS将传统的VS分型进行优化,具有客观、易行、重复性好的优点。[Objective]To explore the application value of magnifying endoscopy combined with blue laser imaging(ME-BLI)in patients with early gastric neoplastic lesions.[Methods]Eighty-eight patients in Endoscopy center of Guilin Medical University from July 2019 to June 2020 were enrolled.They firstly received conventional white light endoscopy(WLE),then ME-BLI and targeted biopsy.According to the pathological results,there were 21 cases in early cancer group and 67 cases in non-early cancer group.Comparison of baseline data and ME-BLI signs was performed between the two groups,the area under receiver-operating characteristic curve(AUC)was performed to evaluate prognostic value of each index in early cancer.[Results]The incidences of the demarcation line,irregular microvascular pattern,irregular microsurface pattern in early cancer group were significantly higher than that in the non-early cancer group(P<0.05).The AUC of DL combined MV and MS on gastric early cancer was 0.875,which had high diagnostic value.[Conclusion]ME-BLI is an important diagnostic method for early gastric cancer,the VS classification was optimized by DL combined MV and MS,it has the advantages of strong objectivity,simple and easy to operate,good repeatability.
