先天性唇裂早期手术修复后继发鼻畸形的Ⅱ期整形临床研究

摘　　要：目的探讨先天性唇裂早期手术修复后继发鼻畸形的Ⅱ期整形临床研究。方法选取2017年至2020年于我院接受治疗的先天性唇裂早期手术修复患儿共计90例作为本次研究对象。其中52例患儿接受早期手术修复后继发鼻畸形,并接受Ⅱ期整形术,将其归为Ⅱ期整形组;其余38例患儿归为一次修复组。对比两组术后各项指标、治疗优良率、鼻外形评分以及美观度评分、患儿家属满意率。结果Ⅱ期整形组患儿鼻孔高度、鼻孔宽度、鼻底长度、鼻翼长度、鼻翼唇峰距、鼻翼口角距、鼻偏曲度以及鼻尖偏移度均明显少于一次修复组,两组具有明显统计学差异(P<0.05);Ⅱ期整形组治疗优良率明显高于一次修复组,两组具有明显统计学差异(P<0.05);Ⅱ期整形组患儿鼻外形评分以及美观度评分均高于一次修复组,两组具有明显统计学差异(P<0.05);Ⅱ期整形组患儿家属满意率为96.15%,一次修复组患儿家属满意率为84.21%,两组具有明显统计学差异(P<0.05)。结论先天性唇裂早期手术修复后继发鼻畸形的Ⅱ期整形可明显改善术后相关指标,提高治疗优良率,改善鼻外形以及提升鼻部美观度,获得患儿家属的高度认可。Objective To investigate the clinical study of phase II plastic surgery for nasal deformity secondary to early surgical repair of congenital cleft lip.Methods A total of 90 children with early surgical repair of congenital cleft lip who were treated in our hospital from 2017 to 2020 were selected as the research objects.Among them,52 patients underwent early surgical repair with secondary nasal deformity and received stage II plastic surgery,and were classified as the stage II plastic surgery group;the remaining 38 children were included in the primary repair group.The postoperative indicators,excellent and good rate of treatment,nasal appearance score and aesthetic score,and the satisfaction rate of children's family members were compared between the two groups.Results Nostril height,nostril width,nasal base length,nasal alar length,nasal alar lip peak distance,nasal alar-mouth angle distance,nasal deviation and nasal tip deviation were significantly lower in the stage II plastic surgery group than in the one-time repair group.Statistical difference(P<0.05);the excellent and good rate of treatment in the stage II plastic surgery group was significantly higher than that in the primary repair group,and there was a significant difference between the two groups(P<0.05).It was higher than the onetime repair group,and there was a significant difference between the two groups(P<0.05).The satisfaction rate of the family members of the children in the stage II plastic surgery group was 96.15%,and the satisfaction rate of the family members of the children in the one-time repair group was 84.21%.difference(P<0.05).Conclusions Phase II plastic surgery for nasal deformity secondary to early surgical repair of congenital cleft lip can significantly improve postoperative related indicators,improve the rate of excellent and good treatment,improve the appearance of the nose,and enhance the aesthetics of the nose,which is highly recognized by the families of the children.
