补肾活血方对复发性流产孕鼠Th17/Treg平衡及妊娠结局的影响

摘　　要：目的观察补肾活血方对复发性流产(RSA)孕鼠辅助性T细胞17(Th17)/调节性T细胞(Treg)平衡及相关细胞因子水平和妊娠结局的影响。方法将雌性CBA/J小鼠与雄性DBA/2小鼠按照2∶1比例交配建立RSA模型,成功后将RSA小鼠随机分为模型组、地屈孕酮组和补肾活血方低、中、高剂量组,每组10只;另将雌性CBA/J小鼠与雄性BALB/C小鼠按照2∶1交配,取正常妊娠的10只雌性小鼠作为正常妊娠组。妊娠第1天,正常妊娠组和模型组孕鼠给予生理盐水灌胃,地屈孕酮组孕鼠给予6.18 mg/kg地屈孕酮灌胃,补肾活血方低、中、高剂量组孕鼠分别给予7.83 g/kg、15.67 g/kg、31.34 g/kg补肾活血方灌胃,均每天1次,连续14 d。疗程结束,取各组孕鼠外周血,检测血清中白细胞介素-17(IL-17)、转化生长因子-β(TGF-β)水平,计算IL-17/TGF-β比值;处死孕鼠,计算胚胎吸收率;流式细胞术检测脾脏淋巴细胞中Th17细胞比例、Treg细胞比例,计算Th17/Treg比值。结果与模型组比较,地屈孕酮组及补肾活血方各组孕鼠血清IL-17水平及IL-17/TGF-β比值、胚胎吸收率、脾脏淋巴细胞中Th17细胞比例和Th17/Treg比值均明显降低(P均<0.05),血清TGF-β水平及脾脏淋巴细胞中Treg细胞比例均明显升高(P均<0.05);地屈孕酮组和补肾活血方中剂量组孕鼠血清IL-17水平及IL-17/TGF-β比值、胚胎吸收率、脾脏淋巴细胞中Th17细胞比例和Th17/Treg比值均明显低于补肾活血方低、高剂量组(P均<0.05),血清TGF-β水平及脾脏淋巴细胞中Treg细胞比例均明显高于补肾活血方低、高剂量组(P均<0.05),地屈孕酮组各指标与补肾活血方中剂量组比较差异均无统计学意义(P均>0.05)。结论补肾活血方可能通过调节Th17/Treg平衡及相关细胞因子水平,使母胎免疫耐受,改善小鼠妊娠结局。Objective It is to observe the effects of Bushen Huoxue decoction on the balance of T helper 17(Th17)/regulatory T cells(Treg)and the levels of related cytokines and pregnancy outcomes in pregnant rats with recurrent abortion(RSA).Methods The RSA models were established by mating female CBA/J mice with male DBA/2 mice at a ratio of 2∶1.After successful modeling,the RSA mice were randomly divided into model group,dydrogesterone group and low-dose,medium-dose and high-dose Bushen Huoxue decoction groups,each group had 10 mice;in addition,female CBA/J mice were mated with male BALB/C mice at a ratio of 2:1,and 10 normal pregnant female mice were selected as the normal pregnancy group.On the first day of pregnancy,the pregnant mice in the normal pregnancy group and model group were given normal saline by gavage,the pregnant mice in the dydrogesterone group was given 6.18 mg dydrogesterone by gavage,and the low-dose,middle-dose and high-dose groups of Bushen Huoxue decoction were respectively given 7.83 g,15.67 g,31.34 g Bushen Huoxue decoction by gavage,all once a day,continuously treated for 14 days.At the end of the course of treatment,the peripheral blood of the pregnant mice in each group was collected,the levels of interleukin-17(IL-17)and transforming growth factor-β(TGF-β)in serum were detected,and the ratio of IL-17/TGF-βwas calculated;the pregnant mice were sacrificed,the embryo absorption rate was calculated;the proportion of Th17 cells and Treg cells in the spleen lymphocytes was detected by flow cytometry,and the Th17/Treg ratio was calculated.Results Compared with the model group,the levels of serum IL-17,the ratio of IL-17/TGF-β,the rate of embryo absorption,the proportion of Th17 cells in spleen lymphocytes and the ratio of Th17/Treg of the pregnant mice in the dydrogesterone group and every Bushen Huoxue decoction group were significantly decreased(all P<0.05),the levels of serum TGF-βand the proportion of Treg cells in spleen lymphocytes were significantly increased(all P<0.05);the level of se
