摘  要:目的 探讨右美托咪定对脑膜瘤患者脑源性神经生长因子(BDNF)水平及认知功能的影响。方法 选取2020年1月至2020年12月间锦州市中心医院收治的60例脑膜瘤患者,随机分为观察组和对照组,每组30例。观察组患者麻醉诱导前静脉注射右美托咪定,对照组患者静脉注射生理盐水,比较两组患者不同时间的心率及平均动脉压,比较手术前后血浆中BDNF水平及简易精神状态量表(MMSE)评分变化。结果 观察组患者不同时间的心率及平均动脉压均低于对照组,差异均有统计学意义(均P<0.05)。术前,两组患者血浆BDNF水平和MMSE评分比较,差异无统计学意义(P>0.05)。术后,两组患者血浆BDNF水平比较,差异有统计学意义(P<0.05);MMSE评分比较,差异无统计学意义(P>0.05)。结论 右美托咪定能够降低脑膜瘤患者术中心率和平均动脉压波动,减小手术造成的血浆BDNF降低幅度,对患者认知功能无明显影响,值得推广应用。Objective To analyze the effects of dexmedetomidine on the plasma levels of brain-derived nerve growth factor(BDNF) and cognitive function in patients with meningioma. Methods From January 2020 to December 2020, 60 patients with meningiomas admitted to Jinzhou Central Hospital were selected. They were randomly divided into an observation group and a control group with 30 patients in each group. The observation group were intravenously injected with dexmedetomidine before anesthesia induction and the control group were intravenously injected with saline before anesthesia induction. Heart rate and mean arterial pressure were compared between the two groups at different times points and changes in BDNF and mini-mental state examination(MMSE) scores were compared before and at 24 h after the surgery. Results The heart rate and mean arterial pressure were lower in the observation group than in the control group at different times, and the differences were statistically significant(all P<0.05). Before the operation, there was no significant difference in plasma BDNF level and MMSE score between the two groups(P>0.05). After the operation, there was significant difference in the level of plasma BDNF between the two groups(P<0.05). There was no significant difference in MMSE score(P>0.05). Conclusion Dexmedetomidine can reduce intraoperative heart rate and mean arterial pressure, and reduce the reduction of plasma BDNF caused by surgery in patients with meningioma. At the same time, it has no obvious impact on the cognitive function of patients. It is worthy of popularization and application.
