摘  要:目的探究Er:YAG激光联合iRoot BP Plus用于深龋露髓恒牙直接盖髓术中的效果。方法选取2020年1月—2021年1月我院收治的50例深龋露髓成熟恒牙(50颗患牙),根据治疗方法不同分为观察组25例和对照组25例,直接盖髓术中分别应用Er:YAG激光联合iRoot BP Plus、Er:YAG激光联合氢氧化钙[Ca(OH)_(2)]治疗。比较2组随访12个月内总体疗效、患牙钙化桥形成情况,2组直接盖髓就诊次数和操作时间及随访6个月时龈沟液中炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、转化生长因子-β1(TGF-β1)、白细胞介素-8(IL-8)]水平。结果观察组随访6、12个月时治疗成功率及患牙钙化桥形成率明显高于对照组(P<0.05)。观察组直接盖髓就诊次数明显少于对照组,盖髓操作时间明显短于对照组(P<0.05)。随访6个月时,2组龈沟液中TNF-α、IL-1β、IL-6、TGF-β1、IL-8水平较术前明显降低,且观察组均明显低于对照组(P<0.05)。结论相较于Er:YAG激光联合Ca(OH)_(2),Er:YAG激光联合iRoot BP Plus用于深龋露髓恒牙直接盖髓术可获得更为确切的临床疗效,操作便利,就诊次数少,更利于减轻患者炎症反应及促进患牙钙化桥形成。Objective To investigate the effect of Er:YAG laser combined with iRoot BP Plus in direct pulp capping of mature permanent teeth with carious pulp exposure.Methods A total of 50 patients with mature permanent teeth with carious pulp exposure(50 affected teeth)treated from January 2020 to January 2021 in our hospital were selected.They were randomly divided into observation group(n=25)and control group(n=25)according to different treatment methods,and treated with Er:YAG laser combined with iRoot BP Plus and Er:YAG laser combined with calcium hydroxide[Ca(OH)2],respectively.The overall efficacy and calcification bridge formation of affected teeth were compared between the two groups during 12 months of follow-up,and the frequency of direct pulp capping,duration of operation,and the levels of inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-6(IL-6),transforming growth factor-β1(TGF-β1),interleukin-8(IL-8)]in gingival cervicular fluid were compared between the two groups after 6 months of follow-up.Results The success rate of treatment and the formation rate of calcified bridge in the observation group were significantly higher than those in the control group at 6 and 12 months after follow-up(P<0.05).The frequency of direct pulp capping in the observation group was significantly less than that in the control group,and the duration of operation of direct pulp capping was significantly shorter than that in the control group(P<0.05).The level of TNF-α,IL-1β,IL-6,TGF-β1 and IL-8 in gingival crevicular fluid of two groups at 6 months after follow-up in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Compared with Er:YAG laser combined with Ca(OH)_(2),Er:YAG laser combined with iRoot BP Plus is more effective in direct pulp capping of permanent teeth with carious pulp exposure,with convenient operation and fewer visits,which is more conductive to reducing inflammatory response of patients and promoting calcification bridge forma
