牙周组织再生术联合无托槽隐形矫治对牙周炎伴错颌畸形患者临床疗效、龈沟液炎症反应程度及牙周健康状况的影响

摘　　要：目的 探讨牙周组织再生术(GTR)联合无托槽隐形矫治对牙周炎伴错颌畸形患者疗效、龈沟液炎症反应程度及牙周健康状况的影响。方法 选取2019年1月—2020年4月收治的牙周炎伴错颌畸形120例,据治疗方法不同分为观察组和对照组各60例,观察组予GTR联合无托槽隐形矫治,对照组予GTR联合直丝弓矫治,比较2组治疗前后龈沟液炎性因子,牙周指标[牙周袋探诊深度(PD)、牙龈指数(GI)、龈沟出血指数(SBI)、牙菌斑指数(PLI)、临床附着丧失(CAL)],临床疗效和并发症发生情况。结果 治疗后2组龈沟液白细胞介素-6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)水平降低,且观察组龈沟液IL-6、CRP、TNF-α水平低于对照组(P<0.01)。治疗后2组PD、GI、SBI、PLI、CAL均降低,且观察组治疗后PD、GI、SBI、PLI、CAL低于对照组(P<0.01)。观察组治疗总有效率明显高于对照组,并发症总发生率明显低于对照组(P<0.05)。结论 牙周炎伴错颌畸形患者采用GTR联合无托槽隐形矫治,有利于提高疗效,改善龈沟液炎症反应程度及牙周健康,并可减少并发症的发生。Objective To investigate the effect of periodontal tissue regeneration(PTR)combined with invisible orthodontic treatment without brackets on the efficacy,inflammatory response of gingival crevicular fluid and periodontal health in patients with periodontitis and malocclusion.Methods A total of 120 cases of periodontitis with malocclusion treated from January 2019 to April 2020 were selected.According to different treatment methods,they were divided into an observation group and a control group,with 60 cases in each group.The observation group received PTR combined with invisible orthodontic treatment without brackets,and the control group received PTR combined with straight arch wire for orthodontic treatment.The inflammatory factors in gingival crevicular fluid,periodontal indexes[periodontal pocket probing depth(PD),gingival index(GI),sulcus bleeding index(SBI),plaque index(PLI),clinical attachment loss(CAL)],clinical efficacy and complications before and after treatment were compared between the two groups.Results The levels of interleukin-6(IL-6),C-reactive protein(CRP)and umor necrosis factor-α(TNF-α)in the gingival crevicular fluid of the two groups decreased after treatment,which were lower in the observation group than in the control group(P<0.01).After treatment,PD,GI,SBI,PLI,and CAL in the two groups decreased,which were lower in the observation group than in the control group after treatment(P<0.01).The total effective rate of treatment in the observation group was significantly higher than that in the control group,and the total incidence of complications was significantly lower than that in the control group(P<0.05).Conclusion The use of PTR combined with invisible orthodontic treatment without brackets in patients with periodontitis and malocclusion is conductive to improvement of the curative effect,the inflammatory response of gingival crevicular fluid and periodontal health,and reduction of the incidence of complications.
