老年胃癌组织TSHR蛋白表达与腹腔镜术后预后的相关性研究

摘　　要：目的 探讨老年胃癌组织促甲状腺激素受体(TSHR)蛋白表达与腹腔镜术后预后的相关性。方法 选取2017年8月至2021年2月间陕西省肿瘤医院收治的90例老年胃癌患者,取胃癌病灶组织及癌旁正常组织标本,检测TSHR蛋白表达水平。调查患者腹腔镜术后预后情况,并进行相关性分析。结果 患者病灶组织的TSHR蛋白表达水平为(0.45±0.09),低于癌旁组织的(3.22±0.13),差异有统计学意义(P<0.05)。患者不同病灶直径、淋巴结转移、临床分期和组织学分化的组织中的TSHR蛋白表达水平比较,差异均有统计学意义(均P<0.05)。随访至2021年6月,90例患者死亡18例(20.0%),生存72例。死亡患者病灶组织TSHR蛋白表达水平为(0.33±0.03),低于生存患者的(0.99±0.17),差异有统计学意义(P<0.05)。Spearman分析显示,90例老年胃癌患者病灶组织中TSHR蛋白表达水平与腹腔镜术后预后存在相关性(r=0.67,P<0.05)。结论 老年患者胃癌组织TSHR蛋白呈现低表达,与患者的病理特征与腹腔镜术后预后存在相关性,可作为诊断胃癌与预测预后的有效标志物。Objective To investigate the association between the expression of thyroid-stimulating hormone receptor(TSHR) protein in gastric cancer tissues and the prognosis after laparoscopic surgery in the elderly. Methods From August 2017 to February 2021, 90 elderly patients with gastric cancer diagnosed and treated at Shaanxi Cancer Hospital were selected. The lesion tissue specimens and the normal paracancerous tissues were collected from these patients to detect the expression level of TSHR protein. The prognosis of patients was investigated after laparoscopic surgery and the association was analyzed. Results The expression level of TSHR protein was 0.45±0.09 in the lesion tissues which was lower than 3.22±0.13 in the paracancerous tissues(P<0.05). There were statistically significant differences in the expression levels of TSHR protein in patients of different histological differentiation, clinical stages, lymph node metastasis and lesion diameter(all P<0.05). All patients were followed up until June 2021. Seventy-two patients survived and 18 patients died, and the mortality rate were 20.0%. The expression level of TSHR protein was 0.33±0.03 in the tissues of deceased patients which was lower than 0.99±0.17 of patients survived(P<0.05). Spearman analysis showed that the expression level of TSHR protein tissues were associated with the prognosis in elderly patients with gastric cancer after laparoscopic surgery(P<0.05). Conclusion The expression of TSHR was low in the tissues of elderly patients with gastric cancer. It was associated with the pathological characteristics and the prognosis after laparoscopic surgery. It can be used as an effective marker for the diagnosis and prognosis of gastric cancer.
