摘　　要：目的 分析FLOT方案与SOX方案在晚期胃癌一线化疗中的应用价值及对血清组织多肽特异抗原(TPS)水平的影响。方法 选取2016年1月至2021年1月间江苏省靖江市人民医院收治的86例晚期胃癌姑息性化疗患者进行回顾性研究,按治疗方案不同分为对照组(43例,予以SOX方案化疗)和观察组(43例,予以FLOT方案化疗),观察两组临床疗效、不良反应TPS水平。结果 较之对照组的客观缓解率(ORR)(32.6%),观察组ORR显著升高(51.2%)(P<0.05)。两组化疗耐受性良好。对照组患者手足综合征发生率较高,观察组患者恶心呕吐、白细胞减少、脱发发生率较高(P<0.05)。用药前两组TPS对比无统计学差异,用药后观察组TPS(47.62±5.11)U/L较对照组低(P<0.05)。结论 FLOT方案可提升晚期胃癌一线化疗效果,不良反应可控,亦可促进血清TPS水平下降,值得临床推荐。Objective To analyze the value of FLOT versus SOX regimen first-line chemotherapy for advanced gastric cancer and its effect on the expression of apoptosis related factors. Methods A retrospective investigator study was conducted in 86 patients with advanced gastric cancer undergoing palliative chemotherapy from January 2016 to January 2021. Based on the treatment regimen, 43 patients who were given docetaxel, oxaliplatin, calcium folinate and 5-fluorouracil were included in an observation group(FLOT regimen) and 43 patients who were given tegafur plus oxaliplatin were included in a control group(SOX regimen). The clinical efficacy, tissue peptide specific antigen(TPS) level, and apoptosis related factors were estimated and compared between the two groups. Results The objective remission rate(ORR) increased in the observation group compared with the control group(51.2% vs 32.6%, P<0.05). Chemotherapy tolerance was good in both groups. The incidence of hand-foot syndrome was high in the control group and the incidence of nausea and omitting, leukocytopenia and alopecia was high in the observation group. There was no statistical difference in TPS between the two groups before the treatment(P>0.05). After the treatment, TPS was lower in the observation group(47.62±5.11 U/L) than in the control group(P<0.05). Conclusion FLOT regimen can improve the effect of first-line chemotherapy for advanced gastric cancer, and can also promote tumor cell apoptosis. It is recommended for clinical reference.
