摘　　要：目的 探讨脑星形胶质细胞瘤羧肽酶E(CPE)的表达及其病人预后的关系。方法 选取2011年3月至2012年3月手术切除并经术后病理证实的脑星形胶质细胞瘤86例,免疫组织化学染色检测组织CPE表达水平,根据染色强度分为高表达和低表达。随访截止2022年3月,记录总生存时间(OS)。结果 86例中,毛细胞型(WHO分级Ⅰ级)20例,弥漫型(WHO分Ⅱ级)22例,间变型(WHO分Ⅲ级)26例。WHO分级Ⅲ级星形细胞瘤CPE高表达率[65.00%(13/20)]明显高于WHO分级Ⅰ级[45.45%(10/22)]、Ⅱ级[34.62%(9/26)]星形细胞瘤(P<0.05)。多因素Cox回归分析显示,CPE低表达是星形胶质细胞瘤患存预后不良的独立影响因素(HR=3.000;95%CI 1.274~4.462;P<0.001)。生存曲线分析显示CPE低表达组中位OS[12个月(9~38个月)]明显低于高表达组[18个月(14~44);P<0.05]。结论 随WHO分级升高,星形胶质细胞瘤CPE表达水平逐渐降低,CPE低表达与脑星形胶质细胞瘤不良生存预后有关。Objective To investigate the relationship between the expression of carboxypeptidase E(CPE) in brain astrocytoma tissues and the prognosis of astrocytoma patients. Methods The expression of CPE was detected using immunohistochemical staining in the astrocytoma tissues obtained fron 86 astrocytoma patients who were surgically resected from March 2011 to March 2012. According to the staining intensity, the expression level of CPE was divided into high expression and low expression. Follow-up ended in March2022, and overall survival(OS) was recorded. Results Of these 86 patients, 20 patients were pilocytic astrocytoma(WHO grade Ⅰ), 22diffuse astrocytoma(WHO grade Ⅱ), and 26 anaplastic astrocytoma(WHO grade Ⅲ). The high expression rate of WHO grade Ⅲastrocytoma [65.00%(13/20)] was significantly higher than that of WHO grade Ⅰ [45.45%(10/22)] and grade Ⅱ [34.62%(9/26)]astrocytoma(P<0.05). Multivariate Cox regression analysis showed that low CPE expression was an independent factor for poor survival prognosis of astrocytoma patients(HR=3.000;95% CI 1.274~4.462;P<0.001). Survival curve analysis showed that the median OS [12months(9~38 months)] in the low-expression CPE group was significantly lower than that in the high-expression group [18 months(14-44);P<0.05]. Conclusions With the increase of WHO grade, the expression level of CPE in astrocytoma gradually decreases. Low CPE expression is associated with poor survival prognosis of astrocytoma patients.
