摘 要:目的 探讨伴深覆(牙合)颞下颌关节紊乱病(TMD)患者的盘髁位置变化的磁共振成像(MRI)特征,为临床正畸治疗深覆(牙合)错(牙合)畸形时及时诊断TMD提供帮助。方法 选取2020年1月至2021年12月工作或学习于郑州大学第一附属医院的25例双侧颞下颌关节正常的志愿者纳入对照组;选取同期就诊于郑州大学第一附属医院口腔正畸科的50例符合纳入标准的TMD患者纳入试验组,其中确诊为TMD且伴有深覆牙合的25例患者纳入试验a组,不伴深覆牙合的25例患者纳入试验b组。应用MRI扫描分析各组患者闭口位上关节盘和髁突的位置,应用SPSS 21.0统计软件对测量的数据进行统计学分析。结果 试验a组的患者髁突在关节窝内均向后、上方移动(P<0.05);试验组的关节盘前移位(P<0.05)。结论 伴深覆牙合的TMD患者较不伴深覆(牙合)的TMD患者和正常牙合人群,其颞下颌关节的盘髁位置变化更大。深覆牙合髁突后移的异常关节结构与TMD关系密切。Objective To investigate the magnetic resonance imaging(MRI) features of changes in the position of the disc condyle in patients with deep overbite occlusal temporomandibular joint disorder(TMD),and to provide help for the timely diagnosis of TMD in the clinical orthodontic treatment of deep overbite occlusal malocclusion.Methods A total of 25 healthy volunteers with normal bilateral temporomandibular joints who worked or studied in the First Affiliated Hospital of Zhengzhou University from January 2020 to December 2021 were selected as the control group.There were 50 patients with TMD who met the inclusion criteria from the Department of Orthodontics of the First Affiliated Hospital of Zhengzhou University during the same period were selected as the test group,including 25 patients with TMD and deep overbite occlusion in test group a and 25 patients without deep overbite occlusion in test group b.MRI scanning was used to analyze the position of the upper articular disc and condyle in the closed position of patients in each group,and SPSS 21.0 statistical software was used to statistically analyze the measured data.Results The condyles of test group a moved backward and upward in the articular fossa(P<0.05).The articular disc of test group was displaced anteriorly(P<0.05).Conclusion TMD with deep overbite has more changes in the disc condyle position of temporomandibular joint than TMD without deep overbite and individual normal people.Abnormal joint structure with deep overbite condyles has a close relationship with TMD.
