摘 要:目的调查2018-2020年河南省碘缺乏地区博爱县8~10岁儿童和孕妇的碘营养状况。方法按照《河南省碘缺乏病监测方案》,在博爱县按东、西、南、北、中5个方位各抽取1个乡镇/街道,对8~10岁儿童及孕妇食用盐碘情况、尿碘含量和儿童甲状腺容积进行调查分析。结果2018-2020年博爱县儿童合格碘盐食用率分别为62.7%、56.7%、90.3%,孕妇合格碘盐食用率为60.0%、53.3%、80.0%。儿童尿碘收集649份,尿碘中位数203.1μg/L,不同年份差异有统计学意义(P<0.05)。孕妇尿碘收集320份,尿碘中位数153.0μg/L,不同年份差异有统计学意义(P<0.05)。共检查儿童甲状腺B超441人,甲状腺肿大率3.9%。结论博爱县儿童和孕妇碘营养总体处于合理水平,但孕妇碘营养不足人群占比较大,应坚持食盐加碘为主的综合防治策略并加强孕妇的动态监测和健康教育工作。Objective To investigate the iodine nutrition status among 8-10-year-old children and pregnant women in the iodine deficient areas,Boai county,from 2018 to 2020.Methods According to“Henan Surveillance Program on Iodine Deficiency Disorders”,5 townships/subdistricts,which were randomly selected in Boai county based on the directions,east,west,south,north and middle.Urinary and salt iodine content,and thyroid volume among 8-10-year-old children and pregnant women were investigated and analyzed.Results The consumption rates of qualified iodized salt from 2018 to 2020 were 62.7%,56.7% and 90.3% among children,respectively,and were 60.0%,53.3% and 80.0% among pregnant women,respectively.A total of 649 children’s urine samples were collected,and the median urinary iodine was 203.1μg/L;the difference of median urinary iodine per year was statistically significant(P<0.05).A total of 320 urine salt samples among pregnant women were collected,and the median urinary iodine was 153.0μg/L;the difference of median urinary iodine per year was statistically significant(P<0.05).Thyroid volume among 441 school children was examined,and the goiter rate was 3.9%.Conclusions The iodine nutrition status among children and pregnant women is overall appropriate in Boai county,but the proportion of iodine nutritional insufficiency among pregnant women is relative high.The comprehensive prevention and control strategy of salt iodization should be adhered to,and the dynamic monitoring and health education of pregnant women should be strengthened.
