摘　　要：目的:分析与探讨腮腺少见病变影像表现。方法:回顾性分析2017年8月至2019年4月21例经黄石市中心医院临床手术病理证实经临床手术病理证实的腮腺少见病变的影像资料。结果:基底细胞腺瘤发病率相对较高;肌上皮瘤及肌上皮癌好发于腮腺浅叶;脂肪瘤具有特异性影像表现;淋巴上皮囊肿为单发含囊性成分病灶,其与表皮样囊肿、皮样囊肿鉴别较为困难;良性淋巴上皮病变表现双侧腮腺弥漫性病变;淋巴瘤与转移瘤鉴别较难,但转移瘤多半有原发肿瘤病史;高分化鳞癌无特异性影像表现,通过临床表现鉴别良恶性病变;涎腺导管癌与弥漫性腮腺炎性病变较难鉴别,术前易被误诊为腮腺炎而导致二次扩大手术。结论:部分少见腮腺病变仍具有相对特异性影像学表现,术前完善影像检查对疾病的诊断及鉴别诊断具有重要意义。Objective:Analyzed and discussed the imaging manifestations of rare parotid gland lesions.Methods:It has been used the method to retrospectively analyze the imaging data from 21 cases of rare parotid gland lesions confirmed by clinical surgery and pathology in Huangshi Central Hospital from August 2017 to April 2019.Results:The incidence of basal cell adenoma is relatively high;Myoepithelial neoplasia and myoepithelial carcinoma occur in the parotid gland;Lipoma has specific imaging findings;Lymphatic epithelial cyst is a single cystic component lesion,which is difficult to distinguish from epidermoid cyst and dermoid cyst;Benign lymphoid epithelial lesions show bilateral diffuse lesions of the parotid gland;Lymphoma and metastasis are more difficult to distinguish,but metastatic tumors have a history of primary tumors.Highly differentiated squamous cell carcinoma without specific imaging findings,identifying benign and malignant lesions through clinical manifestations;Parotid ductal carcinoma and diffuse mumps lesions are difficult to identify,and are easily misdiagnosed as mumps before surgery,resulting in secondary enlargement surgery.Conclusion:Some rare parotid lesions still have relatively specific imaging findings.Preoperative perfect imaging examination is of great significance for the diagnosis and differential diagnosis of diseases.
