摘 要:目的分析非肿块强化(non⁃mass enhancement,NME)乳腺癌锥光束乳腺CT(cone beam breast computed tomography,CBBCT)特征与人表皮生长因子受体2(human epidermal growth receptor 2,HER2)表达的相关性。方法选择2019年7月至2019年12月于广西医科大学附属肿瘤医院经病理证实为NME乳腺癌的69例患者为研究对象,并分为HER2阳性组和HER2阴性组,采用χ^(2)检验比较两组患者CBBCT特征的差异。结果CBBCT图像分析显示,两组强化分布均以段样分布为主;HER2阳性组局灶性分布比例低于HER2阴性组(3.1%vs 24.3%,P=0.031)。强化特征方面,HER2阳性组以集群卵石样强化为主;HER2阳性组不均匀强化比例低于HER2阴性组(25.0%vs 48.6%,P=0.043),但簇状小环形强化比例高于HER阴性组(15.62%vs 0,P=0.018)。钙化分布方面,HER2阳性组集群分布比例低于HER2阴性组(17.4%vs 47.7%,P=0.032),但段样分布比例高于HER2阴性组(39.1%vs 9.5%,P=0.023)。可疑钙化形态方面的特征两组差异均无统计学意义(均P>0.05)。结论CBBCT中NME强化分布及强化类型、钙化分布的影像学特征与HER2表达具相关性,CBBCT可能作为NME乳腺癌预后评估及临床治疗方案选择的辅助手段。Objective To analyze the correlation between cone beam breast computed tomography(CBBCT)features and human epidermal growth receptor 2(HER2)expression in non⁃mass enhancement(NME)breast cancer.Methods A total of 69 patients with NME breast cancer,who were pathologically confirmed in Guangxi Medical University Cancer Hospital from July 2019 to December 2019,were selected as the research subjects,and divided into HER2 positive and HER2 negative groups.The differences of CBBCT features between the two groups were compared by theχ^(2) test.Results The CBBCT image analysis demonstrated that the enhancement distribution in both groups was dominated by the segment⁃like distribution;the proportion of the focal area distribution in HER2 positive group was lower than that of HER2 negative group(3.1%vs 24.3%,P=0.031).In terms of enhancement characteristics,the HER2 positive group was dominated by the clustered pebble⁃like enhancement;the proportion of heterogeneous enhancement in HER2 positive group was lower than that of HER2 negative group(25.0%vs 48.6%,P=0.043),while the proportion of the clustered small ring enhancement was higher than that of HER2 negative group(15.62%vs 0,P=0.018).In terms of calcification distribution,the proportion of cluster distribution in HER2 positive group was lower than that of HER2 negative group(17.4%vs 47.7%,P=0.032),but the proportion of the segment⁃like distribution was higher than that of HER2 negative group(39.1%vs 9.5%,P=0.023).There was no statistical signifi⁃cance in the characteristics of calcification suspicious morphology between two groups(All P>0.05).Conclusions The imaging fea⁃tures of the NME enhancement distribution,enhancement type,and calcification distribution in CBBCT are correlated with HER2 ex⁃pression.CBBCT may be the auxiliary for prognosis evaluation and clinical treatment plan selection of NME breast cancer.
