摘　　要：目的分析全自动血细胞分析仪检测分类的白细胞散点图(WBC-Diff通道),总结急性白血病异常散点图的特点,探讨异常白细胞散点图对急性白血病的辅助诊断价值,尝试利用散点图特点对急性白血病快速做出初步分型诊断。方法回顾性分析22例急性白血病患者全血标本散点图,标本涂片镜检,根据病种进行对比,分析其共有特点。结果XN-2000全自动血细胞分析仪散点图对初步诊断急性髓系原始粒细胞a型白血病、急性早幼粒细胞白血病、急性淋巴细胞白血病均具有一定临床意义,尤其对急性早幼粒细胞白血病具有较高敏感性和特异性。结论异常白细胞散点图辅助诊断急性白血病具有积极临床意义,利用白细胞散点图可在外周血涂片检查结果之前提供重要信息,快速、简便,早期发现并可初步判断分型。Objective To analyze the leukocyte scatter diagram(WBC-Diff channel)of the automatic blood cell analyzer detection classification,summarize the abnormal scatter diagram characteristics of various types of acute leukemia,in order to explore the value of abnormal leukocyte scatter diagram for the auxiliary diagnosis of acute leukemia patients,and try to make a rapid preliminary typing diagnosis of acute leukemia using the scatter diagram characteristics.Methods A total of 22 whole blood specimens from patients suffering from acute leukemia were selected and their scatter plots were retrospectively analyzed,and all 22 specimens were smeared,microscopically examined and compared according to disease types to find out their common features.Results The scatter plot of XN-2000 automatic hematocrit analyzer had certain clinical significance for the preliminary diagnosis of acute myeloid primitive granulocyte a leukemia,acute promyelocytic leukemia,and acute lymphoblastic leukemia,especially acute promyelocytic leukemia,which had a high sensitivity and specificity.Conclusion Abnormal leukocyte scattergram is an important indicator for detecting acute leukemia and helps to determine its typing.Especially when the workload is high,the use of leukocyte scattergram can provide important information before peripheral blood smear examination,making quick,easy and early detection and preliminary typing.
