姑息放疗联合阿帕替尼在Ⅳ期胰腺癌中的应用效果

摘　　要：目的:研究姑息放疗联合阿帕替尼在Ⅳ期胰腺癌(pancreatic cancer,PC)中的应用效果。方法:选取2018年1月-2019年1月在首都医科大学石景山教学医院接受治疗的60例Ⅳ期PC患者。按照随机数字表法将其划分为对照组和研究组,各30例。对照组采用姑息放疗,研究组在此基础上口服阿帕替尼。比较两组临床疗效、生存情况,治疗前后疼痛评分,胰腺外分泌功能情况及不良反应。结果:研究组完全缓解(CR)、部分缓解(PR)占比,客观缓解率(objective response rate,ORR)、疾病控制率(disease control rate,DCR)均高于对照组,疾病进展(PD)占比低于对照组(P<0.05)。治疗后,研究组视觉模拟评分法(visual analogue scale,VAS)评分低于对照组(P<0.05)。治疗后,所有患者随访时间为4~30个月,期间随访率100%,经统计,研究组无进展生存期(progression-free-survival,PFS)、总生存期(overall survival,OS)均长于对照组(P<0.05)。治疗后,研究组粪弹力蛋白酶-1(fecal elastase-1,FE-1)水平高于对照组,重度胰腺外分泌功能不全率低于对照组(P<0.05)。研究组高血压、骨髓抑制、恶心呕吐及放射性炎症发生率均低于对照组(P<0.05)。结论:姑息放疗联合阿帕替尼治疗Ⅳ期PC效果良好,可改善临床症状,减少不良反应,提高其生存周期,值得临床推广。Objective:To study the application effect of palliative radiotherapy combined with Apatinib on stage Ⅳ pancreatic cancer (PC).Method:A total of 60 patients with stage Ⅳ PC who were treated in Shijingshan Teaching Hospital of Capital Medical University from January 2018 to January 2019 were selected.They were divided into control group and study group according to the random number table method,with 30 cases in each group.The control group received palliative radiotherapy,and the study group took Apatinib orally on this basis.The clinical efficacy,survival condition,pain score,pancreatic exocrine function before and after treatment and adverse reactions were compared between the two groups.Result:The proportions of complete remission (CR),partial remission (PR),objective response rate (ORR) and disease control rate (DCR) in the study group were higher than those in the control group,and the proportion of disease progression (PD) was lower than that in the control group (P<0.05).After treatment,the visual analogue scale (VAS) score of the study group was lower than that of the control (P<0.05).After treatment,all patients were followed up for 4 to 30 months,and the follow-up rate was 100%.According to statistics,the progression-free-survival (PFS) and overall survival (OS) of the study group were longer than those of the control group (P<0.05).After treatment,the level of fecal elastase-1 (FE-1) in the study group was higher than that in the control group,and the rate of severe pancreatic exocrine insufficiency was lower than that in the control group (P<0.05).The incidences of hypertension,bone marrow suppression,nausea,vomiting and radiation inflammation in the study group were lower than those in the control group (P<0.05).Conclusion:Palliative radiotherapy combined with Apatinib is effective in the treatment of stage Ⅳ PC.It can improve clinical symptoms,reduce adverse reactions,and improve they life cycle.It is worthy of clinical promotion.
