摘　　要：目的探讨B超联合肿瘤标志物对胃肠癌腹膜转移的诊断价值。方法收集126例胃肠癌患者的病历资料,所有患者入院后均行癌胚抗原(CEA)、糖类抗原125(CA125)、糖类抗原19-9(CA19-9)检测和腹膜B超检查。以病理诊断腹膜转移为金标准,采用Kappa一致性检验比较B超检查与肿瘤标志物检测诊断胃肠癌腹膜转移与病理结果的一致性。结果126例胃肠癌患者经病理检查诊断腹膜转移18例,B超检查、肿瘤标志物检测以及B超联合肿瘤标志物检测异常分别为20例、55例和17例。B超联合肿瘤标志物检测的准确度及与病理结果的一致性均高于B超及肿瘤标志物单独检测。结论B超联合肿瘤标志物在胃肠癌腹膜转移中有重要的诊断价值。Objective To investigate the diagnostic value of B-ultrasonography combined tumor marker detection in peritoneal metastasis of gastrointestinal cancer.Method Clinical data of 126 patients with gastrointestinal cancer was collected,and all patients underwent carcinoembryonic antigen(CEA),carbohydrate antigen 19-9(CA19-9),carbohydrate antigen 125(CA125)and peritoneal B-ultrasonography detection.Using pathological diagnosis as the gold standard,the consistency of B-ultrasonography and tumor markers with pathological results in diagnosing peritoneal metastasis of gastrointestinal cancer was compared by Kappa consistency test.Result A total of 18 cases of peritoneal metastasis were diagnosed by pathology among 126 patients with gastrointestinal cancer,and there were 20,55 and 17 cases,respectively diagnosed by B-ultrasonography,tumor markers and combination of B-ultrasonography with tumor markers.The consistency of combined B-ultrasonography and tumor markers with pathological results was higher than that of single detection.Conclusion B-ultrasonography combined tumor marker detection is of great value in diagnosing peritoneal metastasis of gastrointestinal cancer.
