摘　　要：目的分析肿瘤浸润淋巴细胞(TIL)表达在局部晚期乳腺癌新辅助化疗(NAC)疗效的价值。方法选取2017年1月至2021年1月在梧州市红十字会医院经空心针穿刺活检确诊为乳腺浸润性癌的局部晚期乳腺癌(TNM分期为T3及T4期)患者,其中进行NAC后行乳腺癌改良根治术的60例为临床研究对象。对术后取得的标本中TIL表达情况进行分析,并根据其表达情况分为高表达、中表达、低表达三组。采用组织病理学评定标准对NAC疗效进行评定,并探究影响疗效的相关因素。结果NAC治疗的总体疗效为78.33%;化疗疗效与簇分化抗原CD4、CD8表达程度均具有相关性(P<0.05);化疗疗效与叉头框蛋白P3(Fox-P3)表达无相关性(P>0.05)。因变量为化疗疗效,自变量为CD4、CD8,logistic回归分析表明,CD4、CD8与化疗疗效呈正相关,是化疗疗效的促进因素(P<0.05)。结论对局部晚期乳腺癌NAC疗效的预测,TIL中CD4、CD8表达情况具有较高的评定价值,属于疗效评定的独立因子,化疗疗效与表达程度成正比。Objective To analyze the value of tumor infiltrating lymphocytes(TIL)expression in the efficacy of neoadjuvant chemotherapy(NAC)for locally advanced breast cancer.Methods From January 2017 to January 2021,patients with locally advanced breast cancer diagnosed as invasive breast cancer(TNM stages were T3 and T4)by core needle biopsy in Wuzhou Red Cross Hospital were selected,and 60 cases who underwent modified radical mastectomy for breast cancer after NAC were selected for the clinical study.According to the expression,they were divided into three groups,high expression,medium expression and low expression.The curative effect of NAC was evaluated by histopathological evaluation criteria,and the influencing factors affecting the curative effect were explored.Results The total effective rate of NAC was 78.33%.The efficacy of chemotherapy was correlated with the expression of cluster differentiation antigens CD4 and CD8(P<0.05).There was no correlation between chemotherapy efficacy and the expression of forkhead box protein P3(Fox-P3)(P>0.05).The dependent variable was the efficacy of chemotherapy,and the independent variables were CD4,CD8 and Fox-P3.The logistic regression analysis showed that the CD4 and CD8 were positively correlated with the efficacy of chemotherapy and were the promoting factors of the efficacy of chemotherapy.Conclusion In the prediction of NAC efficacy in locally advanced breast cancer,the expression of CD4 and CD8 in TIL is of high value,which is an independent factor for evaluation of curative effect,and the efficacy of chemotherapy is directly proportional to the degree of expression.
