宫腔术后分别联合左炔诺孕酮宫内节育系统与地屈孕酮治疗子宫息肉的效果及对术后复发率的影响

摘　　要：目的对比子宫息肉患者接受宫腔手术治疗后联合地屈孕酮与左炔诺孕酮宫内节育系统(曼月乐)的效果及降低术后复发率的作用。方法选择2020年3月至2021年12月于新沂市人民医院接受宫腔手术治疗的70例子宫息肉患者进行研究,采取单双号形式分为对照组(单号,35例)与观察组(双号,35例),对照组患者术后联合地屈孕酮治疗,观察组患者术后联合曼月乐治疗,比较两组患者手术前后子宫内膜厚度变化,统计两组患者术后6个月月经量正常占比及疾病复发率。结果两组患者术前检测子宫内膜厚度比较,差异无统计学意义(P>0.05);术后3、6个月观察组患者子宫内膜厚度均低于对照组(P<0.05)。两组患者术后6个月月经量正常占比比较,差异无统计学意义(P>0.05);观察组患者疾病复发率低于对照组(P<0.05)。结论子宫息肉患者宫腔镜术后联合曼月乐治疗能够有效减缓子宫内膜增长速度,预防疾病复发,提倡运用推广。Objective To compare the effect of Dydrogesterone or Levonorgestrel-releasing Intrauterine System(Mirena)after uterine cavity surgery in patients with uterine polyps and the effect of reducing postoperative recurrence rate.Methods A total of 70 patients with uterine polyps who received uterine cavity surgery in Xinyi People’s Hospital from March 2020 to December 2021 were selected for the study,and divided into control group(odd number,35 cases)and observation group(even number,35 cases)in the form of odd and even numbers.The control group was treated with Dydrogesterone after surgery,and the observation group was treated with Mirena after surgery.The changes of endometrial thickness of patients before and after surgery in the two groups were detected and compared,and the proportion of patients with normal menstrual flow and disease recurrence rate at 6 months after surgery were counted between the two groups.Results There was no significant difference in endometrial thickness between the two groups before surgery(P>0.05).The endometrial thickness of the observation group was lower than that of the control group at 3 months and 6 months after surgery(P<0.05).There was no significant difference in the proportion of patients with normal menstrual flow at 6 months after surgery between the two groups(P>0.05).The disease recurrence rate of the observation group was lower than that of the control group(P<0.05).Conclusion Combined Mirena treatment after hysteroscopy in patients with uterine polyps can effectively slow down the growth rate of endometrial and prevent disease recurrence,worthy of promotion.
