摘  要:目的 探讨B族Ⅰ型清道夫受体(SR-B1)外显子8基因变异及HDL-C水平与老年冠心病发病风险的关系。方法 选取我院2020年4月~2022年4月收治的220例老年冠心病患者(研究组)以及同期185例健康体检者(对照组)。采用TaqMan法检测SR-B1外显子8基因多态性,分析SR-B1外显子8基因多态性、HDL-C水平与冠心病发病风险的关系,采用logistic回归分析基因型与冠心病危险因素间的相互作用。结果 SR-B1外显子8基因存在3种基因型:CC、CT、TT,研究组C和T等位基因频率分别为74.32%和25.68%,对照组C和T等位基因频率分别为82.43%和17.57%。研究组的TT基因型、T等位基因频率均高于对照组(P<0.05,P<0.01)。研究组CC基因型的HDL-C水平高于CT、TT基因型,CC基因型的LDL-C水平低于CT、TT基因型(P<0.05)。研究组高血压、糖尿病比例、LDL-C水平显著高于对照组,HDL-C显著低于对照组,差异有统计学意义(P<0.05,P<0.01)。多因素logistic回归分析显示,SR-B1外显子8基因(OR=0.581,95%CI:0.411~0.820,P=0.002)、高血压(OR=1.904,95%CI:1.137~3.188,P=0.014)、糖尿病(OR=1.774,95%CI:1.093~2.878,P=0.020)、HDL-C(OR=0.495,95%CI:0.320~0.764,P=0.002)、LDL-C(OR=1.960,95%CI:1.063~3.613,P=0.031)均是冠心病发病的影响因素。结论 SR-B1外显子8基因多态性及血清HDL-C水平是老年冠心病发病风险的影响因素,其中SR-B1外显子8基因C→T突变可能引起血清HDL-C水平的降低。Objective To investigate the relationship of genetic variation in exon 8 of scavenger receptor class B type 1(SR-B1) and high-density lipoprotein-cholesterol(HDL-C) level with the risk of coronary heart disease in the elderly patients.Methods A total of 220 elder patients with coronary heart disease(study group) and 185 healthy subjects(control group) admitted to our hospital from April 2020 to April 2022 were recruited in this study.TaqMan method was used to detect the polymorphism of SR-B1 exon 8 gene, and the relationship of the polymorphism and HDL-C level with the risk of coronary heart disease was analyzed.Logistic regression analysis was used to analyze the interactions between genotypes and risk factors for coronary heart disease.Results There were 3 genotypes in SR-B1 exon 8 gene: CC,CT and TT.The C and T allele frequencies of the study group were 74.32% and 25.68%,respectively, and were 82.43% and 17.57%,respectively in the control group.The TT genotype and T allele frequencies in the study group were significantly higher than those in the control group(P<0.05,P<0.01).The HDL-C levels of the patients carrying CC genotypes were higher than those of CT and TT genotypes, and the LDL-C levels of CC genotypes were lower than those of CT and TT genotype(P<0.05).The proportions of hypertension and diabetes mellitus and LDL-C levels were significantly higher, while HDL-C level was obviously lower in the study group than the control group(P<0.05,P<0.01).Logistic regression analysis showed that SR-B1 exon 8 gene(OR=0.581,95%CI:0.411-0.820,P=0.002),hypertension(OR=1.904,95%CI:1.137-3.188,P=0.014),diabetes(OR=1.774,95%CI:1.093-2.878,P=0.020),HDL-C(OR=0.495,95%CI:0.320-0.764,P=0.002),and LDL-C(OR=1.960,95%CI:1.063-3.613,P=0.031) were the influencing factors of the risk of coronary heart disease.Conclusion Polymorphism of SR-B1 exon 8 gene and serum HDL-C level are the influencing factors for risk of coronary heart disease in the elderly people.The C→T mutation in SR-B1 exon 8 gene may cause the decrease of serum H
