摘　　要：目的:研究乳腺区段切除术联合腋窝淋巴结清扫术对老年乳腺癌患者的影响。方法:回顾性分析徐州医科大学第二附属医院2014年12月-2019年5月收治的老年乳腺癌患者80例,依据手术方式分为对照组(40例)与试验组(40例),对照组给予改良根治术治疗,试验组给予乳腺区段切除术联合腋窝淋巴结清扫术治疗。比较两组的术后并发症情况、远期疗效及生存质量,比较术前术后两组的体力状况。结果:术后试验组并发症发生率为7.50%,低于对照组的35.00%(P<0.05);术后3个月试验组各项生存质量评分均高于对照组(P<0.05);术后1、3年试验组体力状况评分均高于对照组(P<0.05);两组术后1、3年的生存率、复发率及转移率比较,差异无统计学意义(P>0.05)。结论:乳腺区段切除术联合腋窝淋巴结清扫术可减少老年乳腺癌患者术后并发症的发生,对提高患者的生存质量及体力状况有积极性的影响。Objective:To study the effect of segmental mastectomy combined with axillary lymph node dissection of elderly breast cancer patients.Method:A total of 80 elderly breast cancer patients treated in the Second Affiliated Hospital of Xuzhou Medical University from December 2014 to May 2019 was retrospectively analyzed and they were divided into control group (40 cases) and experimental group (40 cases) according to the operation method.The control group was treated with modified radical mastectomy,and the experimental group was treated with segmental mastectomy combined with axillary lymph node dissection.The postoperative complications,quality of life and long-term effect of the two groups were compared after operation,and the physical condition and of the two groups were compared before and after operation.Result:The incidence of postoperative complications in the experimental group was 7.50%,lower than 35.00% in the control group (P<0.05).Three months after operation,the quality of life scores in the experimental group were higher than those in the control group (P<0.05).At 1,3 years after operation,the physical condition scores in the experimental group were higher than those in the control group (P<0.05).There were no significant differences in the 1 year and 3 years survival rate,recurrence rate and metastasis rate between the two groups (P>0.05).Conclusion:Segmental mastectomy combined with axillary lymph node dissection can reduce the incidence of postoperative complications in elderly breast cancer patients,and has a positive impact on improving the quality of life and physical status of patients.
