通过有限元分析直线通路微创开髓洞型的临床应用效果及预后影响

摘　　要：目的:通过有限元分析方法探讨直线通路微创开髓洞型的临床应用效果及预后影响。方法:选择2021年1-6月东南大学附属中大医院溧水分院口腔内科收治的80例因牙周病或正畸拔除上颌第一前磨牙的患者,使用显微CT数据行三维重建后,于三维模型中模拟直线通路微创开髓洞型、桁架开髓洞型和传统开髓洞型的根管和开髓治疗。完成建模后进行有限元分析,记录斜向与纵向负载模式下,四种模型牙颈部von Mises应力峰值,并进行预后分析。结果:斜向与纵向负载模式下,直线通路微创开髓洞型、桁架开髓洞型与传统开髓洞型腭侧与颊侧牙颈部von Mises应力分布峰值均高于模拟未治疗组(P<0.05);传统开髓洞型腭侧与颊侧牙颈部von Mises应力分布峰值均高于桁架开髓洞型与直线通路微创开髓洞型(P<0.05);桁架开髓洞型与直线通路微创开髓洞型腭侧与颊侧牙颈部von Mises应力分布峰值比较,差异均无统计学意义(P>0.05)。经直线通路微创开髓洞型治疗后,共出现2例并发症,发生率为2.50%,经对症处理后,均明显好转,预后良好。结论:通过有限元分析,直线通路微创开髓洞型能够保护根管治疗后牙体力学性能,且预后良好,值得推广。Objective:To explore clinical application effect and prognosis impact of minimally invasive pulp opening type through straight line access by finite element analysis method.Method:From January and June 2021,80 patients with maxillary first premolars extracted due to periodontal disease or orthodontics were selected from the Department of Stomatology of Lishui Branch of Zhongda Hospital Affiliated to Southeast University,after three-dimensional reconstruction using micro CT data,the root canal and pulp opening treatment of straight line access minimally invasive pulp opening,truss pulp opening and traditional pulp opening were simulated in the three-dimensional model.After modeling,finite element analysis was performed to record the von Mises stress peak of four models under oblique and longitudinal loading modes,and the prognosis was analyzed.Result:In the oblique and longitudinal loading modes,the von Mises stress distribution peaks value of palatal and buccal teeth in the straight line access minimally invasive pulp opening,truss pulp opening and traditional pulp opening type were higher than those in the simulated untreated group(P<0.05),the von Mises stress distribution peaks in the palatal and buccal teeth of traditional pulp opening type were higher than those of the truss pulp opening and straight line access minimally invasive pulp opening(P<0.05),there were no significant differences in the von Mises stress distribution peak value of palatine and buccal teeth between truss opening hole and straight line access minimally invasive pulp opening(P>0.05).After minimally invasive pulpotomy through straight line access,complications occurred in 2 cases,the incidence rate was 2.50%,after symptomatic treatment,both cases were significantly improved,and the prognosis was good.Conclusion:Through finite element analysis,the straight line access minimally invasive pulp opening can protect the mechanical properties of teeth after root canal treatment,and the prognosis is good,it’s worthy of promotion.
