摘　　要：目的探讨腹腔镜下根治术对结直肠癌患者免疫功能、内脏蛋白及预后的影响。方法选取2017年1月-2019年6月收治的结直肠癌107例,按照手术方式不同将其分为观察组56例和对照组51例。观察组采用腹腔镜下根治术治疗,对照组采用开腹手术治疗。比较2组手术相关指标、免疫功能指标、内脏蛋白及预后情况。结果观察组手术时间、术中出血量、首次排气时间和住院时间均短于或少于对照组(P<0.05)。治疗后,CD3+、CD4+、CD8+和转铁蛋白、视黄醇结合蛋白、白蛋白、前白蛋白水平2组均较治疗前降低,但观察组高于对照组(P<0.05)。观察组2和3年生存率高于对照组(P<0.05)。结论腹腔镜下根治术可降低对结直肠癌患者免疫功能、内脏蛋白的影响,提高生存率。Objective To investigate the effect of laparoscopic radical resection on the immune system,visceral proteins and prognosis of the patients with colorectal cancer(CRC).Methods From January 2017 to June 2019,a total of 107 patients with CRC were enrolled and divided into two groups according to different surgical methods,the observation group(n=56)and the control group(n=51).The observation groupwas treated with laparoscopic radical resection,and the control group was treated with laparotomy.The operation-related indicators,immune function indicators,visceral proteins and the prognosis were compared between the two groups.Results The duration of operation,intraoperative bleeding,first exhaust time and length of hospitalization in the observation group were shorter or less than those in the control group(P<0.05).After treatment,the levels of CD3+,CD4+,CD8+,transferrin,retinal-binding protein,albumin,and prealbumin in both groups were lower than those before treatment,but the indicators were higher in the research group than in the control group(P<0.05).The 2-and 3-year survival rates of the research group were higher than those of the control group(P<0.05).Conclusion Laparoscopic radical resection can effectively reduce the impact of surgery on patients'immune function and visceral proteins,and improve the survival rate of patients.
