摘  要:目的分析miR-155、CD1d在胃腺癌患者外周血中的表达,并分析二者与预后的相关性。方法纳入马鞍山市人民医院2016年1月至2018年12月收治的晚期胃腺癌患者90例为研究对象(研究组),同期纳入早期胃腺癌患者90例为对照组。采用实时定量聚合酶链式反应(PCR)法检测各研究对象血清miR-155表达水平。流式细胞仪检测外周血单个核细胞(PBMCs)CD1d表达的细胞百分数。碳水化合物抗原(CA)199、CA724水平用全自动化学发光免疫分析仪检测。ROC分析外周血中miR-155、CD1d水平对晚期胃腺癌的诊断价值。采用Pearson法分析miR-155与CD1d及二者与CA199、CA724的相关性。用Kaplan-Meier法分析miR-155、CD1d表达与晚期胃腺癌患者预后的关系。多因素Cox回归分析影响晚期胃腺癌预后的因素。结果与对照组比较,研究组血清中miR-155、CA199、CA724水平较高,PBMCs中CD1d表达的细胞百分数较低,差异均有统计学意义(P<0.05)。ROC曲线显示,血清miR-155联合PBMCs中CD1d表达的细胞百分数诊断晚期胃腺癌的曲线下面积(AUC)为0.894,其敏感度、特异度分别为83.00%、86.70%。Pearson分析显示,血清miR-155水平与CA199、CA724呈正相关(P<0.05),与CD1d呈负相关(P<0.05)。PBMCs中CD1d表达的细胞百分数与CA199、CA724呈负相关(P<0.05)。外周血中miR-155、CD1d表达与晚期胃腺癌TNM分期、淋巴结转移、组织学分化、远端转移有关(P<0.05)。Kaplan-Meier法显示,miR-155低表达组晚期胃腺癌患者OS显著长于miR-155高表达组,CD1d高表达组晚期胃腺癌患者OS显著长于CD1d低表达组。多因素分析表明,淋巴结转移、远端转移、miR-155高表达、CD1d低表达是影响晚期胃腺癌患者预后不良的危险因素(P<0.05)。结论晚期胃腺癌患者血清中miR-155高表达,PBMCs中CD1d低表达,miR-155、CD1d可预测晚期胃腺癌患者预后不良。Objective To analyze the expressions of miR-155 and CD1d in peripheral blood of patients with gastric adenocarcinoma,and to analyze their correlation with prognosis.Methods Ninety patients with advanced gastric adenocarcinoma who were admitted to Maanshan People’s Hospital from January 2016 to December 2018 were included as the research objects(study group),and 90 patients with early-stage gastric adenocarcinoma were included as the control group.The expression level of serum miR-155 of each research object was detected by real-time quantitative polymerase chain reaction(PCR).The percentage of CD1d-expressing cells in peripheral blood mononuclear cells(PBMCs)was detected by flow cytometry.The levels of carbohydrate antigen(CA)199 and CA724 were detected by automatic chemiluminescence immunoassay.The diagnostic value of peripheral blood miR-155 and CD1d levels in advanced gastric adenocarcinoma was analyzed by ROC.Pearson’s method was used to analyze the correlation between miR-155 and CD1d,and their relationship with CA199 and CA724.Kaplan-Meier method was used to analyze the relationship between the expression of miR-155 and CD1d and the prognosis of patients with advanced gastric adenocarcinoma.Multivariate Cox regression was used to analyze the prognostic factors for advanced gastric adenocarcinoma.Results Compared with the control group,the serum levels of miR-155,CA199 and CA724 in the study group were higher(P<0.05),and the percentage of cells expressing CD1d in PBMCs was lower(P<0.05).The ROC curve showed that the area under the curve(AUC)of serum miR-155 combined with the percentage of cells expressing CD1d in PBMCs for the diagnosis of advanced gastric adenocarcinoma was 0.894,and its sensitivity and specificity were 83.00%and 86.70%,respectively.Pearson analysis showed that the serum miR-155 level were positively correlated with CA199 and CA724(P<0.05),and negatively correlated with CD1d(P<0.05).The percentage of cells expressing CD1d in PBMCs was negatively correlated with CA199 and CA724(P<0.05).T
