经脐单孔与多孔腹腔镜在子宫肌瘤剥除术中的应用效果研究

摘　　要：目的观察单孔与多孔子宫肌瘤剥除术治疗子宫肌瘤的效果。方法选取本院收治的164例子宫肌瘤患者为对象,根据随机数字表法分为两组,单孔组63例接受单孔子宫肌瘤剥除术,多孔组101例接受多孔子宫肌瘤剥除术。比较两组的围术期指标、术后24h疼痛视觉模拟评分法(VAS)评分、体象障碍量表(BIS)评分、切口美观满意度(CS)评分、术后24 h发热情况、并发症发生情况及住院总费用。结果两组的手术时间、术中出血量、术后血红蛋白(HGB)下降水平比较,差异无统计学意义(P>0.05);单孔组的术后住院时间短于多孔组,差异有统计学意义(P<0.05)。单孔组的术后24h VAS评分、术后BIS评分均低于多孔组,CS评分、满意度均高于多孔组,差异有统计学意义(P<0.05)。单孔组出现术后24 h发热1例、并发症1例、住院总费用(15673.0±4.2)元,与多孔组的4例、2例、(13452.0±3.2)元比较,差异无统计学意义(P>0.05)。结论单孔腹腔镜子宫肌瘤剥除术较多孔腹腔镜更有优势,不仅可减轻患者术后疼痛、缩短住院时间,还具有突出的美容效果,疗效理想,安全性可靠,值得在临床推广使用。Objective To observe the effect of single and porous myomectomy in the treatment of uterine fibroids.Methods A total of 164 patients with uterine fibroids in our hospital were selected and divided into two groups according to the random number table method.63 patients in the single-port group underwent single-port myomectomy,and 101 patients in the multi-port group underwent multi-port myomectomy.The perioperative indicators,pain visual analogue scale(VAS)score 24 h after operation,body image disorder self-evaluation questionnaire(BIS)score,cosmetic score(CS),fever 24 h after operation,complications and total hospitalization cost were compared between the two groups.Results There were no significant differences in operation time,intraoperative blood loss and postoperative hemoglobin(HGB)level between the two groups(P>0.05);the postoperative hospital stay in the single-hole group was shorter than that in the multi-hole group,and the difference was statistically significant(P<0.05).The VAS score and BIS score at 24 h after operation in the single-hole group were lower than those in the multi-hole group,and the CS score and satisfaction degree were higher than those in the multi-hole group,and the differences were statistically significant(P<0.05).There was 1 case of fever,1 case of complication,and total hospitalization cost(15673.0±4.2)yuan in the single-hole group,compared with 4 cases,2 cases and(13452.0±3.2)yuan in the multi-hole group,there were no significant differences(P>0.05).Conclusion Single-port laparoscopic myomectomy has the advantage of more-ports laparoscopy.It can not only improve the pain symptoms,but also has a positive effect,a short hospital stay and reliable safety.Moreover,it has outstanding cosmetic effect after surgery,which is very worthy of clinical application and promotion.
