县医院护士参与“智慧医养结合服务”意愿与期待的质性研究

摘　　要：目的深入了解县医院护士参与智慧医养结合服务的意愿及期待,为农村老年人开展智慧医养结合服务提供参考依据。方法采用目的抽样和滚雪球抽样法,选取15名县医院护士进行半结构式访谈,以描述性现象学研究法分析转录稿,并采用Colaizzi七步分析法归纳主题。结果共提炼出4个主题及所属的8个亚主题:服务意愿强烈;认可服务意义(促使就医便捷、减缓家庭负担);实施阻碍繁多(政策环境因素、老年人自身因素、服务主体因素);提出服务期待(服务形式适老化、社会支持全面化、运行机制健全化)。结论县医院护士认可智慧医养结合服务且参与意愿较高,但农村智慧医养结合实施存在较多阻碍,建议加强智慧医养结合在基层的宣传及政策推广力度,从服务形式、社会支持及运行机制等方面构建服务供给体系,以促使其顺利运行。Objective To understand the willingness and expectation of county hospital nurses to participate in intelligent medical-nursing combination services,and provide reference for rural elderly to carry out intelligent medical and nursing services.Methods A total of 15 nurses were selected by purposeful sampling and snowball sampling methods and conducted by semi-structured interviews.The transcripts were analyzed by descriptive phenomenological research method and the topics were summarized by Colaizzi's seven-step analysis.Results A total of 4 themes and 8 subthemes were extracted,such as strong willingness,recognition of the significance(facilitating access to convenient services,reducing the burden on families),numerous obstacles to implementation(policy environment factors,elderly people's own factors,service subject factors),and expectations(ageing-friendly service forms,comprehensive social support,and sound operation mechanisms).Conclusion The county hospital nurses recognize the intelligent medical-nursing combination service and have high willingness to participate in.However,there are many obstacles,and government administrator should strengthen the publicity and policy promotion of intelligent medical-nursing combination service to build a service supply system in terms of service forms,social support and operating mechanisms,so as to promote it ruined smoothly.
