不同亚型轻型脑卒中静脉溶栓疗效分析

摘　　要：目的比较轻型脑卒中(mild ischemic stroke,MIS)患者重组组织型纤溶酶原激活剂(recombinant tissue-type plasminogen activator,rt-PA)静脉溶栓后的疗效。方法选取江苏大学附属医院经过治疗的272例MIS患者,分为静脉溶栓组(93例)和未溶栓组(179例),根据发病后3个月的改良Rankin评分(modified Rankin scale,mRS)判断预后。评价预后良好患者中急性卒中治疗Org 10172试验(trial of Org 10172 in acute stroke treatment,TOAST)分型的结果。结果静脉溶栓组患者良好结局的比例显著高于未溶栓组(P<0.05)。大动脉粥样硬化(large-artery atherosclerosis,LAA)患者良好结局的比例显著高于未溶栓组(P<0.05)。结论静脉溶栓可显著提高MIS患者3个月后良好预后的比例,尤其是LAA型患者。Objective To compare the efficacy of intravenous thrombolysis with recombinant tissue-type plasminogen activator(rt-PA)in patients with mild ischemic stroke(MIS).Methods A total of 272 MIS patients treated in the Affiliated Hospital of Jiangsu University were selected and divided into intravenous thrombolysis group(93 cases)and no thrombolysis group(179 cases).The prognosis was judged according to the modified Rankin scale(mRS)3 months after the onset.Outcomes of trial of Org 10172 in acute stroke treatment(TOAST)classification in patients with favorable prognosis were evaluated.Results The proportion of patients with good outcomes in the intravenous thrombolysis group was significantly higher than that in the no thrombolysis group(P<0.05).The proportion of good outcomes in patients with large-artery atherosclerosis(LAA)was significantly higher than that in the no thrombotic group(P<0.05).Conclusion Intravenous thrombolysis can significantly increase the proportion of patients with MIS who have a good prognosis after 3 months,especially in patients with LAA type.
