全视网膜激光光凝术联合复合式小梁切除术治疗新生血管性青光眼患者的效果

摘　　要：目的:探讨全视网膜激光光凝术(PRP)联合复合式小梁切除术治疗新生血管性青光眼(NVG)患者的效果。方法:选取2019年8月至2020年6月该院收治的88例NVG患者进行前瞻性研究,按照随机数字表法分成对照组、观察组,各44例。对照组采用复合式小梁切除术治疗,观察组在对照组的基础上联用PRP治疗。比较两组手术前后视野缺损程度、视网膜神经纤维层(RNFL)厚度、眼压水平、视力水平以及NVG复发率、并发症发生率。结果:观察组术后6个月视野缺损值小于对照组,NVG复发率低于对照组,RNFL厚度大于对照组,视力水平高于对照组,差异均有统计学意义(P<0.05);术后1周、6个月,观察组眼压水平均低于对照组,术后6个月,两组眼压水平均高于术后1周,但观察组低于对照组,差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:PRP联合复合式小梁切除术治疗NVG患者,可减小视野缺损值,提升RNFL厚度,改善眼压、视力水平,降低复发率,效果优于单用复合式小梁切除术治疗。Objective:To explore effects of panretinal laser photocoagulation(PRP)combined with complex trabeculectomy in treatment of patients with neovascular glaucoma(NVG).Methods:88 NVG patients admitted to this hospital from August 2019 to June 2020 were selected for the prospective study,and were divided into control group and observation group according to the random number table method,44 cases in each group.The control group was treated with complex trabeculectomy,while the observation group was treated with PRP on the basis of that of the control group.The visual field defect,the optic nerve fiber layer(RNFL)thickness,the intraocular pressure level,the visual acuity level,the NVG recurrence rate,and the incidence of complications were compared between the two groups before and after the surgery.Results:The visual field defect value of the observation group at 6 months after the surgery was lower than that of the control group,the NVG recurrence rate was lower than that of the control group,the RNFL thickness was greater than that of the control group,the visual acuity level was higher than that of the control group,and the differences were statistically significant(P<0.05).1 week and 6 months after the surgery,the intraocular pressure level of the observation group was lower than that of the control group;6 months after the surgery,the intraocular pressure level of the two groups was higher than that 1 week after the surgery,but that of the observation group was lower than that of the control group;and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:PRP combined with complex trabeculectomy in the treatment of the NVG patients can reduce the visual field defect value,increase the RNFL thickness,improve the intraocular pressure and visual acuity levels,and reduce the recurrence rate.Moreover,it is superior to single complex trabeculectomy.
