强脉冲光与眼睑熏蒸分别联合睑板腺按摩治疗睑板腺功能障碍相关干眼患者的效果比较

摘　　要：目的:比较强脉冲光与眼睑熏蒸分别联合睑板腺按摩治疗睑板腺功能障碍(MGD)相关干眼患者的效果。方法:选取2018年4月至2020年12月该院收治的80例MGD相关干眼患者进行前瞻性研究,按照随机数字表法分为对照组和观察组各40例。对照组采用眼睑熏蒸联合睑板腺按摩治疗,观察组采取强脉冲光联合睑板腺按摩治疗,比较两组治疗前后眼表疾病指数量表(OSDI)评分、睑板腺分泌功能评分(MGYSS)、标准干眼症状评估问卷(SPEED)评分、非侵入泪膜破裂时间(NITBUT)、泪液分泌试验(SIT)评分和不良反应发生率。结果:治疗后,两组OSDI、SPEED、SIT评分均低于治疗前,且观察组低于对照组,两组MGYSS评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);治疗后,两组NITBUT均长于治疗前,且观察组长于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:强脉冲光联合睑板腺按摩治疗MGD相关干眼患者可提高MGYSS和MGYSS评分,以及降低OSDI、SPEED和SIT评分,优于眼睑熏蒸联合睑板腺按摩治疗效果。Objective:To observe effects of intense pulsed light and eyelid fumigation combined with meibomian gland massage in treatment of meibomian gland dysfunction(MGD)-related dry eye.Methods:80 patients with MGD-related dry eye admitted to this hospital from April 2018 to December 2020 were selected for the prospective study,and were divided into control group and observation group according to the random number table method,40 cases in each group.The control group received eyelid fumigation combined with meibomian gland massage,while the observation group received intense pulsed light combined with meibomian gland massage.The ocular surface disease index(OSDI)score,the Meibomian gland secretion(MGYSS)score,the standard dry eye evaluation questionnaire(SPEED)score,the non-invasive tear breakup time(NITBUT)level,the Schirmer secretion test(SIT)score,and incidence of adverse reaction were compared between the two groups before and after the treatment.Results:After the treatment,the scores of OSDI,SPEED and SIT in the two groups were lower than those before the treatment,and those of the observation group were lower than those of the control group;the MGYSS scores in the two groups were higher than those before the treatment,and that of the observation group was higher than that of the control group;and the differences were statistically significant(P<0.05).After the treatment,the levels of NITBUT in the two groups were longer than those before the treatment,that of the observation group was longer than that of the control group,and the difference was statistically significant(P<0.05).Further,the incidence of adverse reactions in the observation group was 5.00%,compared with 7.50%in the control group,the difference was not statistically significant(P>0.05).Conclusions:Intense pulsed light combined with meibomian gland massage can improve the MGYSS and MGYSS scores,and reduce the OSDI,SPEED and SIT scores in the patients with MGD-related dry eye.Moreover,it is superior to eyelid fumigation combined with meibomian gland ma
