氩激光周边虹膜成形术联合超声乳化及房角黏连分离术治疗闭角型青光眼合并白内障患者的效果

摘　　要：目的:观察氩激光周边虹膜成形术联合超声乳化及房角黏连分离术治疗闭角型青光眼合并白内障患者的效果。方法:选取2017年8月至2020年8月该院收治的80例闭角型青光眼合并白内障患者进行前瞻性研究,按照抽签法分为对照组和观察组各40例。对照组采用超声乳化联合房角黏连分离术治疗,观察组采用氩激光周边虹膜成形术联合白内障超声乳化及房角黏连分离术治疗,比较两组治疗效果、手术前后视力(裸眼视力、最佳矫正视力)水平、眼压水平、前房参数(前房深度、房角开放度和房角黏连度)水平和术后1个月并发症发生率。结果:观察组治疗总有效率为95.00%(38/40),高于对照组的77.50%(31/40),差异有统计学意义(P<0.05);术后3个月,观察组裸眼视力和最佳矫正视力水平均高于对照组,眼压水平低于对照组,差异有统计学意义(P<0.05);术后3个月,观察组前房深度、房角开放度均高于对照组,房角黏连度低于对照组,差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:氩激光周边虹膜成形术联合超声乳化及房角黏连分离术治疗闭角型青光眼合并白内障患者可提高治疗总有效率和视力水平,改善前房参数水平,降低眼压水平,效果优于超声乳化联合房角黏连分离术治疗。Objective:To observe effects of argon laser peripheral iridoplasty combined with phacoemulsification and goniosynechialysis in treatment of patients with angle-closure glaucoma complicated with cataract.Methods:A prospective study was performed on 80 patients with angle-closure glaucoma and cataract admitted to this hospital from August 2017 to August 2020,and they were divided into control group and observation group according to the lottery method,40 cases in each.The control group was treated with phacoemulsification combined with goniosynechialysis,while the observation group was treated with argon laser peripheral iridoplasty combined with phacoemulsification and goniosynechialysis.The treatment effects,the levels of the visual acuity(uncorrected visual acuity,best corrected visual acuity),the intraocular pressure and the anterior chamber parameters(anterior chamber depth,anterior chamber angle and chamber angle adhesion degree)before and after the surgery,and the incidence of complications 1 month after the surgery were compared between the two groups.Results:The total effective rate of the observation group was 95.00%(38/40),which was higher than the control group of 77.50%(31/40),and the difference was statistically significant(P<0.05).3 months after the surgery,the naked visual acuity and the best corrected visual acuity of the observation group were higher than those of the control group,the intraocular pressure level was lower than that of the control group,and the differences were statistically significant(P<0.05).3 months after the surgery,the anterior chamber depth and the anterior chamber angle of the observation group were higher than those of the control group,the chamber angle adhesion degree was lower than that of the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:Argon laser peripheral iridoplasty combined with phacoemulsification and goniosynechial
