基于辨证治疗脑型肝豆状核变性的多维结局指标的优劣解距离理想点法研究

摘　　要：目的:运用优劣解距离法(TOPSIS)对4中心120例脑型肝豆状核变性患者的临床疗效结局指标进行综合评价研究,为临床规范治疗肝豆状核变性提供理论依据。方法:基于前期在临床实践中对脑型肝豆状核变性疾病特点及治疗方式的认知,结合专家问卷调查建立的递阶层次结构模型及指标权重,运用TOPSIS法对4中心120例脑型肝豆状核变性患者的临床结局指标(驱铜期间症状急性加重、UWDRS量表评估、症状体征积分、生活质量等)进行综合评价,使用模糊语言进行评价同时进行量化,计算指标权重并评分。结果:治疗组综合评价指数C值高于对照组,更接近于最优方案,临床疗效优于对照组。结论:TOPSIS法能够区分治疗效果,适宜用来评价具有多个临床疗效指标的方案,适用于中药参与治疗的多维临床结局指标的综合评价研究。中西结合治疗脑型肝豆状核变性的临床疗效明显优于单用西药治疗。Objective:To comprehensively evaluate and study the clinical efficacy outcome indexes of 120 patients with cerebral hepatolenticular degeneration in 4 centers by technique for order preference by similarity to ideal solutiont(TOPSIS),so as to provide a theoretical basis for standard clinical treatment of hepatolenticular degeneration.Methods:Based on the cognition of disease characteristics and treatment methods of cerebral hepatolenticular degeneration in clinical practice in the earlier stage,and combined with the hierarchical structure model and index weight which were established by expert questionnaire,the clinical outcome indexes of 120 patients with cerebral hepatolenticular degeneration in 4 centers were comprehensively evaluated by TOPSIS,such as acute exacerbation of symptoms during copper removal,UWDRS scale evaluation,scores of symptom and physical sign,quality of life,etc.Then the above indexes were evaluated by fuzzy language and quantified,and the index weight was calculated and scored.Results:The comprehensively evaluated index C value of treatment group was higher than that of control group,which was closer to the optimal scheme,and the clinical efficacy of treatment group was better than that of control group.Conclusion:TOPSIS method can distinguish the therapeutic effect,and is suitable for evaluating the scheme with multiple clinical efficacy indexes and for comprehensive evaluation and study on clinical multi-dimensional outcome indexes of TCM participating in treatment.The clinical efficacy of integrated traditional Chinese and western medicine on cerebral hepatolenticular degeneration is significantly better than that of simple western medicine.
