摘 要:目的 探讨脑小血管病(CSVD)患者血清脑源性神经营养因子(BDNF)、神经肽Y (NPY)水平及小血管病(SVD)评分与认知功能状况的相关性。方法选取2019年1月至2020年12月大同市第五人民医院收治的203例CSVD患者,根据是否发生血管性认知障碍(VCI)分为观察组(76例,出现认知障碍)和对照组(127例,未发生认知障碍)。对比两组血清BDNF、NPY水平及SVD评分,观察三项指标对CSVD患者发生VCI的预测价值。结果观察组患者血清BDNF水平明显低于对照组,血清NPY水平及SVD评分高于对照组(P<0.05)。Logistic回归显示,血清BDNF、NPY水平及总SVD评分是CSVD患者发生VCI重要影响因素(P<0.05)。CSVD患者血清BDNF水平与MoCA评分呈显著正相关,血清NPY水平、总SVD评分与MoCA评分呈负相关(P<0.05)。血清BDNF、NPY水平及总SVD评分联合预测VCI的AUC为0.908,灵敏度为77.6%,特异度为92.9%。结论血清BDNF、NPY水平及总SVD评分与CSVD患者是否发生VCI密切相关,联合检测三项指标有助于早期预测VCI的发生。Objective To investigate the correlation between the levels of brain-derived neurotrophic factor(BDNF),neuropeptide Y(NPY),small vessel disease(SVD) score and cognitive function in patients with SVD.Methods A total of 203 patients with cerebral small vascular disase(CSVD) admitted to Datong Fifth People’s Hospital from January 2019 to December 2020 were selected and divided into the observation group(76 cases,with cognitive impairment) and the control group(127 cases,without cognitive impairment) according to the occurrence of vascular cognitive impairment(VCI).Serum BDNF and NPY levels and SVD scores were compared between the two groups.The predictive values of the three indicators for VCI were observed in patients with CSVD.Results The level of serum BDNF in observation group was significantly lower than those in control group,the level of NPY and SVD score were higher than that in control group(P<0.05).Logistic regression showed that serum BDNF and NPY levels and total SVD score were important influencing factors for the occurrence of VCI in patients with CSVD(P<0.05).The level of serum BDNF was significantly positively correlated with MoCA score in patients with CSVD,the level of serum NPY and total SVD score were negatively correlated with MoCA score(P<0.05).The combined prediction of BDNF NPY and total SVD score for VCI was 0.908,with the sensitivity of 77.6% and specificity of 92.9%.Conclusions Serum BDNF and NPY levels and total SVD score are closely related to the occurrence of VCI in patients with CSVD.Combined detection of the three indicators can help to predict the occurrence of VCI in early stage.
