摘　　要：目的:探讨血清正五聚蛋白3(PTX3)在老年前列腺癌(PCa)患者内分泌治疗预后中的价值。方法:选取我院收治的132例接受内分泌治疗的老年PCa患者,统计患者的一般临床资料并ELISA法检测治疗前血清中PTX3水平;根据中位值将患者分为高PTX3组和低PTX3组,比较两组临床病理特征及治疗后疾病进展和生存状况,同时采用单因素及多因素Cox回归分析PTX3是否是生存期的影响因素。结果:高PTX3组和低PTX3组患者的TNM分期、骨转移情况及PSA水平存在统计学差异(P<0.05)。随访截止时高PTX3组进展为去势抵抗性PCa的比例显著高于低PTX3组(P=0.023);高PTX3组的无进展生存期(PFS)和总生存期(OS)明显短于低PTX3组(P<0.05);PTX3水平、TNM分期、骨转移及Gleason评分是PFS和OS的独立危险因素(P<0.05)。结论:高PTX3水平是前列腺癌患者内分泌治疗预后不良的独立危险因素,可作为临床预后评估参考性指标之一。Objective:To investigate the prognostic value of pentraxin 3(PTX3) in endocrine therapy in elderly patients with prostatic cancer(PCa).Methods:The clinical data of 132 patients with PCa receiving endocrine therapy from our hospital were studied. The general clinical data of the patients and the level of PTX3 in the serum before treatment were detected by ELISA. According to the median value, the patients were divided into high PTX3 group and low PTX3 group. The clinicopathological features, disease progress and survival status after treatment were compared between the two groups.Single factor and multivariate Cox regression analysis was used to analyze whether PTX3 was the influencing factor of survival time.Results:There were significant differences(P<0.05)in TNM stage, bone metastasis and PSA level between high PTX3 group and low PTX3 group. At the end of follow-up, the proportion of castrated resistant PCa in high PTX3 group was significantly higher than that in low PTX3 group(P=0.023).The progression-free survival(PFS) and overall survival(OS) in high PTX3 group were significantly shorter than those in low PTX 3 group(P<0.05). PTX3 level, TNM stage, bone metastasis and Gleason score were independent risk factors for PFS and OS.Conclusion:High PTX3 level is an independent risk factor for poor prognosis of prostate cancer patients with endocrine therapy, and can be used as one of the reference indexes for clinical prognosis evaluation.
