摘 要:目的 旨在探讨应用MVFI测量的血管指数VI^(MV)联合BI-RADS在乳腺肿块良恶性鉴别中的应用价值。方法 选取行乳腺超声检查并取得病理结果的130例患者(131个乳腺肿块),分为直径<20 mm和≥20 mm两组,行二维灰阶超声检查获得BI-RADS分类,再进行血管指数VI^(MV)检测。以病理结果为金标准,分析BI-RADS、VI^(MV)和二者联合对乳腺肿块的诊断效能。结果 以病理为金标准,VI^(MV)鉴别乳腺肿块良恶性的cutoff值为2.45;VI^(MV)联合BI-RADS诊断乳腺肿块的AUC、准确度、灵敏度、阴性预测值均较单一BI-RADS有所提高,而特异度和阳性预测值也未明显降低,均在80%以上。结论 血管指数VI^(MV)和BI-RADS联合可有效提高对乳腺肿块的诊断效能,可有效减少不必要的穿刺。Objective This study aimed to investigate the value of VI^(MV)combined with BI-RADS in the diagnosis of breast masses. Methods 131 breast masses in 130 patients underwent both B-mode US and VI^(MV)were included in this study, subgroup analysis was performed according to lesion size.The pathological results were regards as the gold standard to analysis the diagnostic efficacy of BI-RADS, VI^(MV)and VI^(MV)combined with BI-RADS for breast masses. Results The optimal cutoff value of the VI^(MV)for differentiating between malignant and benign breast masses was 2.45.The AUC, accuracy, sensitivity and negative predictive value were of VI^(MV)combined with BI-RADS in the diagnosis of breast masses were higher than those of single BI-RADS, while the specificity and positive predictive value were not significantly reduced, both of which were more than 80%. Conclusions The combination of VI^(MV)and BI-RADS can improve the diagnostic efficiency for breast masses.It has positive effect on reducing FNA of benign breast masses and avoiding over-diagnosis.
