CEUS联合血清3项肿瘤标志物诊断原发性肝癌的价值

摘　　要：目的 探析超声造影(CEUS)、血清3项肿瘤标志物甲胎蛋白(AFP)、甲胎蛋白异质体(AFP-L3)和高尔基蛋白73(GP73)在原发性肝癌(PLC)中的诊断价值。方法 回顾性分析80例疑似PLC患者,根据病理诊断良恶性分为观察组(PLC患者,n=29例)及对照组(良性结节患者51例)。所有患者均进行CEUS检查及血清3项肿瘤标志物检测(AFP、AFP-L3和GP73)。比较两组CEUS相关定量参数、血清肿瘤标志物及两者联合在诊断PLC的价值。结果 与对照组比,观察组平均血流密度(MFD)、血清AFP、AFP-L3和GP73水平均明显增高(P<0.05),达峰时间(t TTP)、峰值强度(PI)均明显降低(P<0.05)。CEUS联合血清3项肿瘤标志物诊断PLC的灵敏度、特异度及准确度均明显高于单一CEUS或血清肿瘤标志物(AFP、AFP-L3和GP73)(P<0.05)。结论 CEUS联合血清AFP、AFP-L3和GP73可用于诊断PLC,具有较高诊断效能,为辅助PLC诊治提供依据。Objective To explore the diagnostic value of contrast-enhanced ultrasonography(CEUS) combined with serum alpha-fetoprotein(AFP), alpha-fetoprotein heteroplasmy(AFP-L3) and Golgi protein 73(GP73) in primary liver cancer(PLC). Methods A total of 80 patients with suspected PLC who visited our hospital were selected retrospectively and divided into the observation group(PLC patients, n=29 cases) and control group(benign nodules patients, n=51), according to the pathological diagnosis. CEUS diagnosis and serum tumor marker diagnosis(AFP, AFP-L3 and GP73) were performed. The CEUS-related quantitative parameters and the serum tumor markers were compared between 2 groups, and the diagnostic value of CEUS combined with serum AFP, AFP-L3 and GP73 was analyzed. Results Compared with the control group, the mean blood flow density(MFD), serum AFP, AFP-L3 and GP73 levels were significantly increased(P<0.05), time to peak(TTP) and peak intensity(PI) were significantly decreased(P<0.05) in the observation group. The sensitivity, specificity and accuracy of CEUS combined with serum AFP, AFP-L3 and GP73 were significantly higher than CEUS or serum tumor markers(AFP, AFP-L3 and GP73)(P<0.05). Conclusions CEUS combined with serum AFP, AFP-L3 and GP73 can be used to diagnose PLC with high diagnostic efficiency, which provides a basis for the diagnosis and treatment of PLC.
