拆除不良修复体后根管再治疗疗效及影响因素分析

摘　　要：目的探讨在患者拆除不良修复体后再使用根管治疗的疗效以及影响因素。方法抽取新疆生产建设兵团第十三师红星医院2019年5月~2022年5月收治的不良修复体患者80例,采用随机数表法分为研究组和对照组,每组40例,其中对照组给予常规治疗,研究组采用根管治疗,对比两组治疗效果、不良反应发生率、治疗前后视觉模拟评分(visual analogue scale,VAS)、牙齿活动度评分、牙周指标等。结果研究组治疗有效率高于对照组(P<0.05);治疗前两组的VAS、牙齿活动度评分比较,差异无统计学意义(P>0.05),治疗后两组的VAS、牙齿活动度评分均降低,研究组变化幅度大于对照组(P<0.05);研究组疼痛、红肿、出血、感染、脓包、咬合障碍、酸胀的不良反应发生率低于对照组,差异无统计学意义(P>0.05);治疗后研究组牙周袋探诊深度(PPD)、临床附着水平丧失(CAL)、探诊出血(BOP)、龈沟出血指数(SBI)均低于对照组(P<0.05)。结论拆除不良修复体后使用根管治疗,可以明显降低患者不良反应发生情况,降低疼痛以及改善牙齿活动度,在临床具有一定参考价值。Objective To investigate the efficacy and infl uencing factors of root canal therapy after removal of bad prosthesis.Methods A total of 80 patients with bad prostheses admitted to the Red Star Hospital of the 13th Division of Xinjiang Production and Construction Corps from May 2019 to May 2022 were selected.According to the diff erent treatment methods,the patients were divided into study group and control group by random number table,with 40 patients in each group.The patients in the control group were given routine treatment,and the patients in the study group were treated with root canal therapy.The treatment effects,incidence of adverse reactions,VAS scores,tooth activity scores and periodortal index before and after treatment were compared between the two groups.Results The treatment effective rate of the study group was higher than that of the control group(P<0.05).There was no significant diff erence in VAS and tooth activity scores between the two groups before treatment(P>0.05).The VAS and tooth mobility scores in both groups were decreased,and the change in the study group was greater than that in the control group(P<0.05).The main adverse reactions in the two groups included pain,redness,hemorrhage,infection,pustules,occlusal disturbance,and soreness in the study group.The incidence of adverse reactions in the study group was lower than that in the control group,and the diff erence was not statistically significant(P>0.05).After treatment,the periodontal pocket probing depth(PPD),clinical attachment level loss(CAL),bleeding on probing(BOP),and sulcus bleeding index(SBI)in the study group were lower than those in the control group(P<0.05).Conclusion The effect of root canal therapy after removing the bad prosthesis can significantly reduce the occurrence of adverse reactions,reduce pain and improve the mobility of tooth,which has a certain reference value in clinical practice.
