摘 要:目的 探讨长骨骨巨细胞瘤(GCT)患者术后复发风险的因素。方法 选择92例长骨GCT患者,均采用手术治疗。收集患者性别、年龄、手术方式、病灶部位、是否合并病理性骨折、Campanicci’s放射线分级及肿瘤直径等资料,术后随访2年,依据患者是否复发将其分为复发组及未复发组,分析术后复发风险因素。结果 92例长骨GCT患者术后复发15例,复发率为16.30%(15/92);两组在病灶刮除+植骨、病灶刮除+骨水泥填充及Campanicci’s放射线分级Ⅲ级占比及肿瘤直径方面比较,差异有统计学意义(P<0.05);两组在性别、病灶部位、是否合并病理性骨折及年龄对比,差异无统计学意义(P>0.05);采用Logistic回归分析显示,手术方式、Campanicci’s放射线分级、肿瘤直径为影响长骨GCT术后复发的独立危险因素(P<0.05且OR≥1)。结论 手术方式、Campanicci’s放射线分级及肿瘤直径是影响长骨GCT患者术后复发的危险因素,术前准确评估Campanicci’s放射线分级,据此选取适宜的手术治疗,能够降低术后复发率,改善患者预后。Objective To explore the risk factors of postoperative recurrence in patients with giant cell tumor of long bone(GCT).Methods 92 patients with long bone GCT were selected and treated with surgery.The data of patients’ gender,age,operation method,focus location,pathological fracture,campanicci’s radiation classification and tumor diameter were collected.The patients were followed up for 2 years.According to whether the patients relapsed,they were divided into recurrence group and non recurrence group,and the risk factors of postoperative recurrence were analyzed.Results Among 92 patients with long bone GCT,15 cases recurred after operation,and the recurrence rate was 16.30%(15/92);There were significant differences between the 2 groups in the proportion of lesion curettage+bone grafting,lesion curettage + bone cement filling,campanicci’s radiation grade Ⅲ and tumor diameter(P0.05);Logistic regression analysis showed that surgical method,campanicci’s radiation grade and tumor diameter were independent risk factors for recurrence after long bone GCT(P<0.05 and or ≥1).Conclusion Surgical method,campanicci’s radiation classification and tumor diameter are the risk factors of postoperative recurrence in patients with long bone GCT.Accurate preoperative evaluation of campanicci’s radiation classification and selection of appropriate surgical treatment can reduce the postoperative recurrence rate and improve the prognosis of patients.
