经内镜支架置入术联合腹腔镜手术治疗左侧大肠癌性梗阻的临床分析

摘　　要：目的:观察经内镜支架置入术联合腹腔镜手术治疗左侧大肠癌性梗阻的效果。方法:选取2018年1月~2019年12月本院92例左侧大肠癌性梗阻患者,随机分为对照组(n=46,结肠灌洗)与观察组(n=46,经内镜支架置入术联合腹腔镜手术),对两组手术相关指标及术后并发症情况进行对比。结果:在手术相关指标对比中,观察组手术时间为(172.13±20.26)min、术中出血量为(25.10±10.05)mL、肛门排气时间为(2.08±0.53)d、住院时间为(14.45±2.19)d,均短于对照组的(230.37±32.17)min、(75.30±20.25)mL、(4.34±0.91)d、(21.38±4.06)d,差异有统计学意义(P<0.05);观察组术后并发症发生率为2.17%,低于对照组的17.39%(χ^(2)=8.021,P<0.05)。结论:内镜支架置入与腹腔镜手术联合治疗左侧大肠癌性梗阻患者,对患者造成的创伤更小,更有利于术后尽早恢复,且对术后常见并发症有积极预防效果,治疗安全性更高。Objective:To observe the effect of endoscopic stent placement combined with laparoscopic surgery in the treatment of left colorectal cancer obstruction.Method:92 patients with left colorectal cancer obstruction in our hospital from January 2018 to December 2019 were randomly divided into the control group(n=46,colonic lavage)and the observation group(n=46,endoscopic stent placement combined with laparoscopic surgery).The relevant indexes and postoperative complications of the two groups were compared.Results:In the comparison of operation-related indicators,the operation time of the observation group was(172.13±20.26)min,the intraoperative blood loss was(25.10±10.05)mL,the anal exhaust time was(2.08±0.53)d,and the hospital stay was(14.45±2.19)d,all shorter than the control group(230.37±32.17)min,(75.30±20.25)mL,(4.34±0.91)d,(21.38±4.06)d,the difference was statistically significant(P<0.05);The incidence of postoperative complications in the observation group was 2.17%,lower than 17.39%in the control group(χ^(2)=8.021,P<0.05).Conclusion:Combination of endoscopic stent placement and laparoscopic surgery in the treatment of patients with left-sided colorectal cancer obstruction causes less trauma to the patient,is more conducive to early postoperative recovery,and has a positive preventive effect on common postoperative complications,and the treatment is safe higher.
