胰岛素联合非诺贝特治疗高甘油三酯血症性胰腺炎疗效观察

摘　　要：目的观察胰岛素和低分子肝素早期联合非诺贝特治疗高甘油三酯血症性急性胰腺炎的临床疗效。方法回顾性分析昆明医科大学第一附属医院重症医学科近4年收治的高甘油三酯血症性急性胰腺炎患者资料,按治疗方法不同,分为胰岛素和低分子肝素治疗的对照组及在对照组基础上早期联合非诺贝特治疗的观察组,比较2组患者的疗效和ICU住院时间。结果2组患者血清甘油三酯达标(TG≤5.65 mmol/L)的时间无明显差异(P>0.05)。治疗后72 h和96 h,TG达标率及TG水平,观察组优于对照组;治疗后72 h、96 h、120 h,液体输入量观察组少于对照组,差异有统计学意义(P<0.05)。ICU住院时间,观察组短于对照组,差异有统计学意义(P<0.05)。结论胰岛素和低分子肝素早期联合非诺贝特可有效减少高甘油三酯血症性急性胰腺炎患者治疗中的血脂反弹和液体输入过量,缩短ICU住院时间。Objective To observe the clinical efficacy of insulin,low molecular weight heparin and fenofibrate in the early treatment of hypertriglyceridemic acute pancreatitis.Methods The clinical data of patients with hypertriglyceridemic acute pancreatitis who were admitted to the intensive care unit of Kunming Medical University in the past 4 years were analyzed retrospectively,according to different treatment method,the patients were divided into two groups of control group of low insulin and molecular weight heparin,and observation group treated with fenofibrate therapy at early stage and based on the control group.Results There was no significant difference in the time of serum triglyceride(TG≤5.65 mmol/L)between the two groups(P>0.05).At 72 and 96 hours after the treatment,the standard rate of TG and the level of TG in the observation group were better than those in the control group,and at 72,96 and 120 hours after treatment,the amount of liquid input in the observation group was less than that in the control group,the difference was statistically significant(P<0.05).The length of stay in ICU of the observation group was shorter than that of the control group(P<0.05).Conclusions Early combination of insulin and low molecular weight heparin with fenofibrate can effectively reduce the blood lipid rebound and excessive fluid input in the treatment of patients with hypertriglyceridemic acute pancreatitis,and shorten the length of ICU stay.
