单侧完全性唇腭裂患者上颌前牵引及扩弓治疗协同效应的有限元分析

摘　　要：目的:探讨单侧完全性唇腭裂患者上颌前牵引及扩弓治疗的协同效应问题。方法:在已建立的单侧完全性唇腭裂上颌骨有限元模型上,应用ANSYS软件分别添加上颌前牵引力,扩弓力及上颌前牵引联合扩弓力,对其上颌矢状向及横向位移进行分析比较。结果:联合组横向位移量明显大于扩弓组,差异具有统计学意义(P<0.01),联合组健患侧位移差值明显小于前牵组,差异具有统计学意义(P<0.05)。结论:单侧完全性唇腭裂患者,联合前牵引治疗可对上颌扩弓产生促进作用,联合扩弓治疗可对上颌前牵引不对称现象起到改善作用。Objective To explore the synergy between maxillary protraction and expansion on patients with unilateral complete cleft lip and palate.Methods On the established finite element model of the maxilla with unilateral complete cleft lip and palate,ANSYS was used to respectively simulate maxillary protraction force,expansion force and maxillary protraction force combined with expansion.The sagittal and transverse displacement of maxilla were analyzed and compared between groups.Results Transverse displacement of combined group was significantly greater than that of expansion group,and the difference was statistically significant(P<0.01).Displacement difference between noncleft side and cleft side of combined group was significantly less than that of protraction group,and the difference was statistically significant(P<0.05).Conclusion Combined with protraction can promote maxillary expansion,and combined with expansion can reduce protraction asymmetry for patients with unilateral complete cleft lip and palate.
