全瓷修复体粘固前排龈在前牙牙体缺损修复治疗中的应用

摘　　要：目的:探究全瓷修复体粘固前排龈对前牙牙体缺损患者的美容修复效果。方法:以2018年5月-2020年2月笔者医院就诊的126例前牙牙体缺损患者为研究对象,采用随机数字表法将其分为对照组(全瓷修复体粘固前不排龈)和观察组(全瓷修复体粘固前排龈),每组63例。比较两组的治疗效果及并发症发生率,观察两组患者治疗前后咬合力、咀嚼效率、菌斑指数(PLI)、出血指数(BI)、牙周探诊深度(PD)、红色美学指数(PES)、白色美学指数(WES)及龈沟液白介素-6(IL-6)、白介素-8(IL-8)水平变化情况。结果:两组治疗成功率、并发症发生率及治疗后咬合力、咀嚼效率比较,差异无统计学意义(P>0.05);治疗后,观察组BI、PD、PLI小于对照组,观察组PES及WES评分高于对照组,IL-6、IL-8水平低于对照组,差异均有统计学意义(P<0.05)。结论:对前牙牙体缺损患者给予全瓷修复体粘固前排龈治疗,可提高牙龈健康水平及美观度,减轻牙周炎症反应。Objective To explore the application of gingival retraction before cementation of all-ceramic restoration in dental esthetic repair of patients with anterior teeth defects.Methods A total of 126 patients with anterior teeth defects were enrolled as the research objects between May 2018 and February 2020.They were divided into control group(no gingival retraction before cementation of all-ceramic restoration)and observation group(gingival retraction before cementation of all-ceramic restoration)by random number table method,63 cases in each group.The curative effect and incidence of complications were compared between the two groups.The changes in bite force,response rate of chewing,plaque index(PLI),bleeding index(BI),periodontal probing depth(PD),pink esthetic score(PES),white esthetic score(WES),and levels of interleukin-6(IL-6)and interleukin-8(IL-8)in gingival crevicular fluid in both groups were observed.Results There was no significant difference in the success rate of bite force,response rate of chewing,treatment or incidence of complications between the two groups(P>0.05).After treatment,BI,PD and PLI were lower than those in control group,PES and WES were higher than those in control group,and levels of IL-6 and IL-8 were lower than those in control group,the differences were statistically significant(P<0.05).Conclusion The application of gingival retraction before cementation of all-ceramic restoration in patients with anterior teeth defects can improve gingival health and aesthetics,relieve periodontal inflammation response and improve chewing function.
